To assess possible coronary vasoconstriction in patients with ischemic heart disease, we measured vascular resistance 12 normal hearts and disease before during the initial 50 seconds of cold pressor test, a stimulus known to produce systemic vasoconstriction. Control was similar two groups, although it did not change vessels, rose by 27 per cent (P less than 0.005) group test. In three flow actually declined despite an increase arterial pressure; four, angina precipitated. Phentolamine...

Vasodilators are considered to be contraindicated in patients with severe aortic stenosis because of concern that they may precipitate life-threatening hypotension. However, vasodilators such as nitroprusside improve myocardial performance if peripheral vasoconstriction is contributing afterload. We determined the response intravenous 25 and left ventricular systolic dysfunction. Patients were included study had been admitted intensive care unit for invasive hemodynamic monitoring heart...

The optimal use of diuretics in decompensated heart failure remains uncertain. We analyzed data from the ADHERE registry to look at impact diuretic dosing. 62,866 patients receiving <160 mg and 19,674 ≥160 furosemide were analyzed. lower doses had a risk for in-hospital mortality, ICU stay, prolonged hospitalization, or adverse renal effects. These findings suggest that future studies should evaluate strategies minimizing exposure high diuretics.

Arteriovenous differences (A-V) of all naturally occurring amino acids, lactate, and oxygen were measured simultaneously with coronary sinus blood flow (CSBF) in 8 normal subjects 11 patients artery disease at rest during pacing stress. Mean values for CSBF myocardial consumptions (MVO2) the two groups similar pacing, although mean MVO2 increased significantly both paced as compared to state. Alanine (ala) was only acid released by myocardium, while glutamic acid(glu) demonstrated uptake....

Background: This study aimed to characterize the single-dose pharmacokinetic (PK) and pharmacodynamic (PD) profile of rivaroxaban 15 mg administered before after dialysis in subjects with end-stage renal disease (ESRD), compare this ESRD that healthy control (creatinine clearance ≥80 ml/min). Methods: was an open-label, single-dose, single-center, parallel-group who had been clinically stable on maintenance...

Abstract Background Postoperative atrial fibrillation ( POAF ) is a well‐recognized complication of cardiac surgery; however, its management remains challenge, and the implementation outcomes various strategies in clinical practice remain unclear. Hypothesis We hypothesize that treatment for variable, it associated with particular morbidity mortality following surgery. Methods compared patient characteristics, operative procedures, postoperative management, between patients without coronary...

Purpose: To determine the effect of resistance exercise training (ET) on glucocorticoid-induced myopathy in heart transplant recipients (HTR), 14 male HTR were randomly assigned to a ET group that trained for 6 months (54± 3 yr old; mean ± SD) or control (51 8 SD). Methods: Fat mass, fat-free and total body mass measured by dual-energy x-ray absorptiometry before 2 after transplantation (Tx), period. The regimen consisted lumbar extension (MedX) performed 1 d·wk-1 variable exercises...

Background: Since most acute decompensated heart failure (ADHF) patients present for hospital care via the emergency department (ED), we sought to determine impact of early ED initiation ADHF-specific therapy, as indicated by nesiritide use, on subsequent outcomes. Methods: We queried Acute Decompensated Heart Failure National Registry (ADHERE®) identify with initial systolic blood pressure >90 mm Hg and negative cardiac biomarkers, hospitalized after...

Two patients developed fever, dermatitis, renal insufficiency, and eosinophilia, three five weeks after beginning therapy with allopurinol. The clinical, laboratory, histologic findings were compatible a severe hypersensitivity reaction. Both required long-term corticosteroid (more than six four months, respectively) to control their symptoms. These observations suggest that allopurinol can produce severe, prolonged, or potentially fatal illness.
