背景.近年,オシメルチニブ投与中にtransient asymptomatic pulmonary opacities(TAPOs)と呼ばれる予後良好の肺病変を高頻度に認めることが報告されている.症例.76歳の男性が肺腺癌に対してオシメルチニブを投与され,良好な治療経過を得ていた.36週目に無症候性の肺浸潤影が出現し,経気管支肺生検の病理学的検査では器質化肺炎に合致する所見であった.呼吸状態の悪化がみられず,患者が継続を強く希望したため,オシメルチニブを慎重に継続したところ,9週間後に浸潤影は消退した.結論.本症例はオシメルチニブ継続投与にも関わらず自然消退した経過からTAPOsが疑われた.TAPOsは重篤な薬剤性肺障害の初期段階との鑑別が難しく,肺癌診療における重要な臨床課題である.

背景.免疫チェックポイント阻害薬では一部の症例で長期奏効例がみられる.しかし,薬剤性肺障害などの免疫関連有害事象も遅発性に生じうる.症例.患者は71歳の男性.2年前から左下葉原発肺腺癌に対してペムブロリズマブを投与され,病変はほぼ消失した.胸部CTで左肺の上葉と下葉に新規陰影が出現し,気管支鏡にて左上葉病変から腺癌細胞を検出した.ペムブロリズマブ休薬後も浸潤影の拡大を認めたため,ステロイド薬を開始したところ,左上葉結節影を除いて浸潤影はほぼ消退した.肺癌再発と遅発性薬剤性肺障害を同時に生じたと考えられた.結論.画像所見だけでは肺癌再発と薬剤性肺障害の鑑別が難しいことがあり,気管支鏡検査による病理学的検討やステロイドへの反応性などを参考にして慎重に判断する必要がある.
