Many people with Parkinson disease (PD) develop motor complications that are uncontrolled by levodopa dose adjustment. Among these patients, it is uncertain which drug class more effective as adjuvant therapy. To compare the long-term effects on patient-rated quality of life adding a dopamine agonist vs reuptake inhibitor (DRI), either monoamine oxidase type B (MAO-B) or catechol-O-methyltransferase (COMT) inhibitor, to therapy for treatment patients PD.This pragmatic semifactorial (2 × 1)...

Accurate diagnosis of the cause parkinsonism during life can be difficult, particularly at presentation, but few studies have described changes in clinical over time and effect applying strict research criteria.Incident patients with a possible/probable degenerative or vascular had standardised assessment yearly intervals thereafter which most likely was recorded without application criteria. Four years after beginning incident period, formal criteria were applied retrospectively using...

Purpose The purpose of this study was to show the viability and performance a shape-based pattern recognition technique applied 123I-N-ω-fluoropropyl-2-β-carbomethoxy-3β-(4-iodophenyl) nortropane single-photon emission computed tomography (FP-CIT SPECT) in patients with parkinsonism. Methods A fully automated tool, based on shape FP-CIT SPECT images, written using Java. Its evaluated compared QuantiSPECT, region-of-interest-based quantitation observer receiver operating characteristic...

Background:
The issue of whether to adopt a "wait and watch" strategy or initiate drug therapy soon after diagnosis in Parkinson's disease (PD) has been the subject some debate. A recent observational study supported early treatment by demonstrating deterioration self-reported health status those left untreated, but not who received therapy. We aimed replicate this observation.
Methods:
People with PD from prospective incidence underwent follow-up yearly clinical assessment...

Background: Cognitive decline is common in Parkinson's disease (PD) but may not be adequately identified by the mini-mental state examination (MMSE), which better suited to Alzheimer's disease. The Parkinson (MMP) a cognitive screening tool designed French specifically for PD. We aimed establish validity and reliability of English language version MMP compared with MMSE. Methods: People various stages PD underwent testing MMSE, was then reference standard battery neuropsychological tests...

Parkinson's disease (PD) is a neurodegenerative disease, the clinical features of which are usually asymmetrical at presentation. This can lead to difficulty in differentiating it from other asymmetric neurological disorders. We present two cases where idiopathic PD was initially misdiagnosed as stroke, leading delay appropriate symptomatic therapy. Physicians involved diagnosis and treatment people with strokes should consider when formulating their differential diagnosis.

Background: Hospital care of older adults with frailty is complex. The post-take ward round (PTWR) a key moment at which this complexity can be managed. However, in the busy environment an acute take it difficult to consistently systematic and address all points suggested by joint college guidelines on round.Checklist methodology was applied attempt improve consistency documentation within Geriatric Assessment Unit.Innovation: A checklist containing 12 items deemed essential effective PTWR...
