Surveillance of infectious episodes in institutionalized elderly men permanently resident on two wards a veterans' hospital was undertaken for 12‐month period. One‐hundred eleven were identified 50 residents (74 per cent). The most frequent infections included lower respiratory tract (incidence 59/100 patient‐years), febrile with no source (43.4), skin and soft tissue (36.5), gastroenteritis (33). Only pneumonia associated significant mortality. A specific etiologic agent seldom indentified...
SUMMARY Three unrelated severe infections with Streptococcus zooepidemicus occurred in England 1985. The first patient developed septic arthritis, which has not been recorded before this organism. second died septicaemia, pneumonia and post-streptococcal glomerulonephritis, the only record so far of nephritis following sporadic S. infection systemic sepsis same patient. third experienced septicaemia during pregnancy but recovered without complications. A likely animal source was found one case.

We determined the validity of using external devices for urine specimen collection from 24 elderly incontinent men residing in a nursing home by collecting three sequential specimens, two with and then one catheterization. The positive predictive value organisms isolated quantitative counts greater than or equal to 10(5) CFU/ml bladder bacteriuria was 86% either sterile clean 93% same organism consecutive specimens. negative present less 90% both when Contamination not influenced whether...
Sixty consecutive patients with central bronchial carcinomas were studied by fibreoptic bronchoscopy. In all forceps biopsy and needle aspiration performed, in 54 brushings obtained. The combination of diagnosed carcinoma a defined cell type 80% patients. Bronchial was the most effective single technique giving cytological diagnosis patients, when three techniques combined positive rate increased to 92%. No major complications occurred using aspiration. Needle particularly helpful sampling...
Abstract: This paper argues that capitalist accumulation requires imperialist expansion, and this expansion creates a "raced" surplus laboring population. The argument proceeds in seven parts: Marx's assertion chapter 25 of Capital capitalism produces an ever-increasing relative population is tenable all but the longest time frames; imperial played important role transition to capitalism, though not for reasons traditionally given; overinvestment rather than increasing organic composition...
AIMS--To determine the expression of intercellular adhesion molecule 1 and 2 (ICAM 2) in transitional cell carcinoma cells before after immunotherapy with Calmette-Guérin bacillus (BCG). METHODS--Frozen sections from 22 untreated bladder carcinomas were immunohistochemically examined monoclonal antibodies to ICAM 2. Urinary cytospin slides made for six patients each clinical instillations which constitute a therapeutic course. These also stained leucocyte function associated antigen (LFA 1)....

The purpose of this study was to increase understanding the experience parenting kindergarten-aged children who are anxious. Twenty-three in-depth interviews were conducted with parents expressed interest in a parent-focused early intervention program for child anxiety offered local elementary school. Key concerns included their children’s separation anxiety, social and oppositional behaviour. child’s identified as stressor on child, parent, family. Parents utilized range responses although...
Episodes of bacteremia identified in a long term care facility over seven and half-year period from July 1984 to December 1991 were reviewed. Twenty-nine episodes identified, rate 4.35/100,000 patient-days. The most common infecting organisms Escherichia coli (11 episodes), Streptococcus pneumoniae (four), Proteus mirabilis (three), Staphylococcus aureus (three) Bacteroides species (two). source was urinary 45% patients, gastrointestinal 17%, pneumonia 14%, skin 14% unknown 10%. overall case...
Two hundred and forty‐three urine specimens from 76 elderly institutionalized residents were obtained for culture, quantitative leukocyte count urinalysis. Significant bacteriuria was present in 153 (63%), including 33 (22%) with more than one organism. Pyuria (greater or equal to 10 leukocytes/mm 3 ) 214 (88%), 116 (97%) single organism bacteriuria, 27 (82%) multiple 71 (80%) without significant bacteriuria. The esterase test had a positive predictive value of 99% pyuria but negative only...
Abstract Decreases in the blood oxygenation level dependent signal are somewhat puzzling for several reasons. The first is synaptic complexity interactive networks, and second involves interplay between oxidative demand perfusion producing change. third reason relativity of both activation deactivation to choice baseline condition. fourth that deactivations could reflect passive shunting activating areas as easily they active inhibition. However, understanding necessary fully understand how...
Summary Cyclospora cayetanensis is the cause of a prolonged diarrhoeal syndrome. In UK most cases are seen in travellers who have returned from countries which organism endemic. The purpose this investigation was to determine incidence C. stool samples submitted district general hospital. A total 6151 stools 5374 different patients were screened for presence over 1-year period using modified Ziehl-Neelsen technique. Oocysts found 7 (0·1%) 4 (0·07%). All four adult had visited known be...
Information was recorded prospectively on 2586 unselected patients with lung cancer seen in hospital during 1981-4. Twenty eight per cent were female and 97% current smokers or ex-smokers. Two thirds aged under 70 at the time of diagnosis. The cell type known 2117 (82%). In 579 women there a larger proportion small carcinoma (29% v 21%) adenocarcinoma (18% 11%) but smaller squamous (38% 52%; p less than 0.01) among men. also significantly younger. Small more common 60 years (p 0.01). Only 47...

Cyclospora cayetanensis is a cause of prolonged diarrhoea, mainly in travellers. Laboratory diagnosis may be achieved by number methods such as the staining faecal smears modified Ziehl-Neelsen (ZN) technique. Safer using this technique have been described for acid fast bacilli and cryptosporidia replacing phenol content carbol fuschin stain with various concentrated detergents. In report was slightly non-concentrated detergent applied to oocysts C cayetanensis. It found that do not ZN...
