症例は69歳,女性.46年前に急性虫垂炎で虫垂切除術を受けていた.3年前より虫垂炎術創近傍に腫瘤を自覚し増大するため当院受診.虫垂炎術創に接する14cm大の腫瘤を触知し,CTで腹壁に石灰化と造影効果を伴う嚢胞性病変を認めた.CEAとCA19-9が高値であったが腹壁腫瘤以外に病変指摘し得ず,遺残虫垂を認めたため虫垂粘液腫瘍を念頭に手術を施行した.遺残虫垂には異常なく,腹壁腫瘤との連続性もなく,それぞれ切除した.腫瘤内部はゼリー状物質で充満し,壁の所々に乳頭状の腫瘍を認め病理組織学的に粘液嚢胞腺癌と診断された.免疫染色でCK7陰性,CK20陽性にて虫垂原発と推定されたが,遺残虫垂に腫瘍は認められず,虫垂炎手術時に術創に粘膜のimplantationが起こり,長期間を経て癌化したものと推察された.本症例のように腹壁に発症した粘液嚢胞腺癌の報告はなく,興味深い症例であったため若干の文献的考察を加え報告した.
