感染性肝囊胞は比較的稀な疾患とされているが画像診断の進歩により診断する機会が増加しており,当院では2009年から2018年の間に23例を経験した.今回は画像検査の所見と囊胞液の検査結果,治療経過について検討した.腹部超音波検査では22例に囊胞内のスラッジエコーを認め,腹部造影CT検査では5例に囊胞壁の造影効果を認めた.腹部MRI検査を14例に行い全例でT2強調画像での囊胞内部の信号低下を認め,拡散強調画像を追加した12例では囊胞内部や囊胞辺縁の拡散低下を認めた.囊胞液の培養は12例が陽性で血液培養も10例で陽性であり,起因菌はKlebsiella pneumoniaeが最多であった.治療は全例に抗菌薬投与と囊胞内容液の穿刺吸引または持続ドレナージを施行して11例は塩酸ミノサイクリンによる硬化療法を追加した.1例はドレナージにて改善が得られず外科手術に至ったが,最終的に全例が軽快退院した.

A 47-year-old man who had undergone subtotal stomach-preserving pancreatoduodenectomy presented with recurrent cholangitis, possibly due to a hepaticojejunostomy anastomosis (HJA) stricture ([Fig. 1]).
