Good综合征是一种原发性免疫缺陷合并胸腺瘤疾病,在1955年被首次发现。典型的特征是胸腺瘤,低丙种球蛋白血症,外周血低或缺少B细胞,CD4+T淋巴细胞减少,CD4+/CD8+T淋巴细胞比值倒置,T细胞有丝分裂原增殖反应减少。临床表现多样,约50%的患者以顽固性腹泻为主要症状。目前关于Good综合征患者出现腹泻的病因还未明确,可能与病原体感染、吸收不良综合征、自身免疫性因素等相关。本文报道了2例厦门大学附属第一医院收治的以腹泻为主要表现的Good综合征患者。2例患者均有胸腺瘤病史,反复肺部感染表现,肠镜检查均发现溃疡性病变,病理提示慢性炎症细胞浸润,肉芽组织形成,无浆细胞或仅可见少量浆细胞浸润。实验室检查表现为低丙种球蛋白血症、外周血T淋巴细胞计数减少、B淋巴细胞减少或缺失、CD4+/CD8+T淋巴细胞的比值倒置。目前主要的治疗措施包括手术切除胸腺瘤、静脉补充丙种球蛋白(IVIG)、针对不同病原体的抗感染处理等,但治疗的总体有效率不高,预后不良。.
