Following the advent of antibiotics, pneumococcal pericarditis has become a rare clinical diagnosis. We discuss 2 cases that were preceded by community-acquired pneumonia. Cultures negative in both patients and diagnosis was made antigen testing polymerase chain reaction (PCR) pericardial fluid. PCR or fluid for pneumococci should be considered with typical history, particularly when culture-negative history prior antibiotics.
