胆嚢結石症は妊婦に合併する消化器外科疾患において急性虫垂炎に次いで多いとされる．症例は27歳女性．糖尿病合併妊娠にて当院産科通院中であった．妊娠21週0日胆石発作のため当院救急外来受診となった．その後も数日おきに発作が頻発したため入院加療とした．食事で発作が誘発されるため経口摂取再開困難であった．中心静脈栄養による長期管理は子宮内胎児発育遅延のリスクがあるため，妊娠24週3日に手術施行した．臍からopen法により1stトロッカーを留置することで安全に気腹し，胆嚢摘出術を施行することができた．術中の気腹圧は8-10mmHgとして管理した．術後は良好に経過し妊娠40週0日2,824gの男児を経膣出産した．妊娠中期は比較的安全な手術が可能であるとされる．妊娠中の胆嚢結石症では症状の再燃率が高率であり，手術を第一選択とすることで胎児と妊婦に対するリスクを最小限にできる可能性がある．
