Clinical evaluation to differentiate the characteristic features of pulmonary fibrosis and emphysema is often difficult in patients with combined (CPFE), but diagnosis important for evaluating treatment options risk acute exacerbation interstitial pneumonia such patients. As far as we know, it first report describing a correlation among clinical, radiological, whole-lung pathological an autopsy cases CPFE patients.Experts retrospectively reviewed clinical charts examined chest computed...

The treatment efficacy of immune checkpoint inhibitor (ICI) and clinical outcomes in patients with non-small cell lung cancer (NSCLC) who develop severe grade pneumonitis (CIP) are unclear. Here, we report on the ICI prognosis NSCLC CIP.In this retrospective cohort study, CIP severity, CIP-related mortality, 71 advanced treated ICIs were evaluated. Data was obtained from patients' medical charts. All observed 22 11 patients, respectively. mortality rate 22.7% (N = 5). An Eastern Cooperative...

A 56-year-old woman, a never-smoker, had postoperative recurrence of anaplastic lymphoma kinase rearranged lung cancer. She achieved partial response to treatment with an tyrosine inhibitor, crizotinib. After the tumor regrowth, crizotinib was switched alectinib; once again observed. At second recurrence, transbronchial needle aspiration right paratracheal node performed, which revealed cytological findings small-cell carcinoma. While cisplatin-irinotecan chemotherapy made reduction some...

Transbronchial lung cryobiopsy (TBLC) has been increasingly utilised to diagnose diffuse parenchymal diseases (DPLDs) and cancers; however, TBLC protocols have not yet standardised the rate of complications associated with this procedure vary widely. Therefore, prospective multicentre observational study investigated safety utility technique in patients localised respiratory diseases. This was conducted at multiple medical centres Japan between July 2018 April 2019. The study's primary...

Background Idiopathic pulmonary fibrosis (IPF) is an interstitial lung disease characterised by chronic fibrosis, and acute exacerbation of IPF (AE-IPF) the leading cause death in patients with IPF. Data on association between body mass index (BMI) prognosis AE-IPF are lacking. This study was performed to evaluate BMI in-hospital mortality who developed using a national inpatient database. Methods Using Japanese Diagnosis Procedure Combination database, we retrospectively collected data...

Objective The number of patients using direct oral anticoagulants (DOACs) has recently increased. However, the association between DOAC use and bronchoscopy (BS) safety not been elucidated. This study examined BS safety. Methods Using data from Japanese Diagnosis Procedure Combination database July 1, 2010, to March 31, 2022, taking DOACs who underwent (n = 603, group) those did 187,827, non-DOAC were identified. Then, 1:4 matched-pair cohort analyses performed based on institution, sex,...

Propionibacterium acnes is thought to be a causative agent of sarcoidosis. Patients with sarcoidosis have circulating immune complexes. We attempted detect P. acnes-derived complexes in sarcoid lesions. We evaluated formalin-fixed and paraffin-embedded lymph node samples from 38 patients 90 non-sarcoidosis (27 necrotizing lymphadenitis, 28 reactive 16 colon cancer, 19 gastric cancer) by immunohistochemistry using anti-human immunoglobulins (IgG, IgA, IgM) complement (C1q C3c) antibodies,...

Immune checkpoint inhibitors have changed the landscape of classic cancer treatment. However, their use is associated with emergence new adverse events. An elderly man rheumatoid arthritis was started on pembrolizumab for newly diagnosed advanced lung cancer. He subsequently developed hemophagocytic lymphohistiocytosis (HLH), which potentially fatal but has not been properly established as an immune inhibition-induced event. We herein report case a patient pembrolizumab-induced HLH.

Benralizumab is a humanized, fucosylated, monoclonal antibody that targets the interleukin 5 (IL-5) α receptor. Several phase III trials have shown benralizumab can significantly reduce incidence of acute exacerbations and improve lung function in patients with severe asthma. However, there paucity data from clinical practice. In this prospective study, we evaluated effectiveness safety for asthma practice. This was prospective, open-label, single-arm, single-center study practice...

Most lung cancer patients present with lesions in both fields and lymphadenopathy. Thus, transbronchial cryobiopsy (TBLC) endobronchial ultrasound-guided needle aspiration (EBUS-TBNA) are commonly performed for diagnosing cancer. However, the adequacy of these samples next-generation sequencing (NGS) analysis remains unclear. This study aimed to compare between TBLC EBUS-TBNA NGS analysis. This retrospective cohort included whose were collected via or analyzed using NGS. Out 46 genes, number...
