大腸癌肝転移の治療方針としては切除可能であれば外科的切除が推奨されている.しかし肝転移切除後にさらに肺転移も発生した際の肺切除に対する手術適応指針として明確なものはない.当施設で行った肝転移切除の既往のある大腸癌肺転移切除30例について臨床病理学的因子および予後を解析し,肺転移巣切除の有用性を検討した.原発巣および肺転移巣切除後の5年生存率はそれぞれ67.5%と41.6%であった.臨床病理因子別による生存率の比較では,多発肺転移例が単発例よりも予後が悪い傾向にあった.肺転移巣切除後24例に肝や肺などに再転移が認められたが,さらに再転移巣切除が行われた群は非切除群に比べ有意に予後良好であった.大腸癌肺転移症例においては,肝転移切除の既往があっても肺転移切除は十分考慮されるべき治療法と考えられた.ただし予後不良傾向である多発肺転移例や高い再転移出現率への対応が今後の課題であると考えられた.

慢性閉塞性肺疾患既往の68歳男性.CTにて右S1に75 mm,右S3に25 mm,右S2に17 mmの腫瘤及び結節を認め,S1病変は気管支鏡検査で扁平上皮癌と診断.病変は右上葉内に限局していた為,胸腔鏡下右肺上葉切除+ND2a-1を施行.病理はS1が腺扁平上皮癌,S3が大細胞神経内分泌癌,S2が大細胞神経内分泌癌の肺内転移であった.#11sリンパ節に腺扁平上皮癌の転移を認め,病期は腺扁平上皮癌がpT3c(pl3)N1M0-IIIA,大細胞神経内分泌癌がpT3(pm1)N0M0-IIBとなった.S1病変は胸壁切離断端陽性であった為58 Gyの術後照射を追加した.稀な組織型の二重肺癌切除例を経験したので報告する.
