TP53 mutants (mutp53) are involved in the pathogenesis of most human cancers. Specific mutp53 proteins gain oncogenic functions (GOFs) distinct from tumor suppressor activity wild-type protein. Tumor-associated macrophages (TAMs), a hallmark solid tumors, typically correlated with poor prognosis. Here, we report non-cell-autonomous mechanism, whereby cancer cells reprogram to supportive and anti-inflammatory state. The colon harboring GOF selectively shed miR-1246-enriched exosomes. Uptake...

Abstract Tumour heterogeneity in primary prostate cancer is a well-established phenomenon. However, how the subclonal diversity of tumours changes during metastasis and progression to lethality poorly understood. Here we reveal precise direction metastatic spread across four lethal patients using whole-genome ultra-deep targeted sequencing longitudinally collected tumours. We find one case surgical bed causing local recurrence, another cross-metastatic site seeding combining with dynamic...

Infection control practices are not uniformly successful in limiting outbreaks of vancomycin-resistant enterococci (VRE). Despite the implementation barrier precautions for VRE-infected patients, nearly one-half inpatients at our center were found to have gastrointestinal colonization by VRE. In an attempt outbreak, we altered antibiotic formulary restricting use cefotaxime and vancomycin adding beta-lactamase inhibitors replace third-generation cephalosporins. The clindamycin was also...

Abstract Muscle Invasive Bladder Cancer (MIBC) has a poor prognosis. Whilst patients can achieve 6% improvement in overall survival with Neo-Adjuvant Chemotherapy (NAC), many do not respond. Body fluid mutant DNA (mutDNA) may allow non-invasive identification of treatment failure. We collected 248 liquid biopsy samples including plasma, cell pellet (UCP) and supernatant (USN) from spun urine, 17 undergoing NAC. assessed single nucleotide variants copy number alterations mutDNA using...

To evaluate the feasibility, safety, and outcome of laparoscopic kidney transplantation (LKT) after retroperitoneoscopic living-donor nephrectomy.Between February 2010 January 2012, a total 217 renal transplantations were performed from living donors by single surgical unit. All nephrectomies approach. Recipient surgery was either laparoscopically (n=72) or conventional open approach (n=145). In LKT group, 5 to 6 cm Pfannenstiel incision placed dropped into abdomen. Renal vessels anastomosed...

Laparoscopic donor nephrectomy has the advantages of less pain, early ambulation and shorter hospitalization compared to open nephrectomy. Kidney recipient surgery is, however, traditionally performed by surgery. Our aim was study feasibility safety laparoscopic kidney transplantation (LKT). After permission from Internal Review Board, LKT in four patients. All kidneys were procured deceased donors. Left used for transplanted left iliac fossa while right standard (OKT). procedures...

There is no uniformly accepted method for detecting colonization with vancomycin-resistant enterococci (VRE). The sensitivities of five culture methods were determined patients known to harbor VRE. Of 189 inpatients, 101 found VRE by at least one method. Three detected fewer than half the cultures. Campylobacter agar identified 70% patients. Enterococcosel broth (containing vancomycin and aztreonam) 88% may be preferred over other media routine surveillance.

Since Partin introduced the analysis of prostate-specific antigen, clinical T-stage and Gleason scores to estimate risk progression in men with localised prostate cancer, our understanding factors that modify this has changed drastically. There are now multiple stratification tools available, including look-up tables, stratification/classification analyses, regression-tree nomograms artificial neural networks. Concurrently, descriptions novel biopsy strategies, imaging modalities biomarkers...

Introduction: The aim of this study was to compare intraoperative and early post-operative outcomes between retzius-sparing robotic-assisted radical prostatectomy (RS-RARP) versus standard approach for RARP (S-RARP). Patients methods: All RARPs by a single surgeon were included divided into two groups: (1) including combined anterior–posterior (S-RARP); (2) posterior (RS-RARP). Allocation based on prostate size location index lesion MRI. Initial follow-up at 6 weeks. Results: Overall, 169...
