【背景】本邦での鍼に関するランダム化比較試験の試みは数少なく、対照群に鍼治療以外の治療法をおいた研究はほとんどない。本邦において、医療制度の中に鍼灸が位置付いてゆくためには、質の高い臨床研究の結果が求められている。今回の臨床試験はこのような状況を踏まえ鍼を受療することの多い「腰痛症」を対象として行われた。【目的】「腰痛症」に対する低周波鍼通電療法の有効性および安全性を経皮的電気刺激法を対照としたランダム化比較試験により検討する。本試験は1995年9月から1996年6月にかけて瀬踏み的になされた第1期の研究を引き継ぐ第2期に相当する探索的なもので, 第3期の確認的な試験へ向けてのデータ収集の意味を持つ。【対象および方法】下肢症状がなく、発症から2週間以上経過した腰痛患者を対象に低周波鍼通電療法 (A群) と経皮的電気刺激法 (T群) の多施設ランダム化比較試験とした。観察期間は2週間、治療回数は5回とした。通電は各群とも1...
【目的】病的共同運動を認める顔面神経麻痺1症例を対象に、非同期的な鍼通電 (100Hz間欠波交互通電) を行い、その有効性について検討した。また、周波数別の鍼通電による変化についても検討した。【方法】鍼治療は表情筋に対する100Hz間欠波交互通電で、EMGを中心とした評価 (EMG振幅、EMG残存持続時間) と顔面神経麻痺スコア、Visual Analogue Scale (VAS) 、Electroneuronography (ENog) による評価とし、治療前後および経時的な変化を観察した。【結果】EMG所見においてEMG振幅に変化は認められなかったが、EMG残存持続時間は治療後および経時的に短縮した。また、VAS、顔面神経麻痺スコア、ENoGはそれぞれ改善した。周波数別の鍼通電によるEMG残存持続時間の変化は、1Hz通電で不変、30Hz通電で延長、100Hz通電で短縮した。【結論】100Hz間欠波交互通電はEMG残存持続時間を短縮させるとともに.病的共同運動の自覚症状を軽減させた。また、顔面神経麻痺スコア、ENoG値も改善することができた。

感染制御の観点から鍼灸師養成学校(鍼灸学校)の実技実習で使用されている鍼の種類や衛生的取扱い方法の指導について把握する目的で，鍼灸学校を対象にアンケートを実施した．2008年12月～2009年3月に全国の鍼灸学校に調査用紙を郵送し，回答を求めた．質問項目は，実習で使用している鍼の種類，鍼の取扱い方法の質問などから構成され，回答は選択形式とした． 回収率は69.9%であった(107/153施設)．実習で使用している鍼の種類について調査したところ，「単回使用毫鍼のみ使用している」が58%，「単回使用毫鍼と再使用可能な毫鍼を併用して使用している」が40%，「再使用可能な毫鍼のみ使用している」が2%であった．しかし，単回使用毫鍼を使用していても，本来の使用目的どおり，単回使用を指導している割合は25%であった．また，施術する際，鍼を直接手指で触れないためにゴム手袋や指サック等を使用しているかを調査した．「常に使用するように指導している」が17%，「使用するかどうかは各教員に任せている」が35%，「使用するようには指導していない」が48%であった．...
末梢性顔面神経麻痺に対する鍼治療の効果を検討するため、日本顔面神経研究会治療効果判定委員会の提唱する基準に基づき、対象の選択を発症して2～3週間以内の新鮮例で, 電気生理学的検査 (Electroneurography : ENoG) による神経変性の程度を診断した上で、顔面運動スコアを用いて、薬物療法と鍼治療の比較、また薬物療法に鍼治療を併用した際の薬物単独療法との回復の違い等について111例の症例に対しretrospective study により治療効果を検討した。その結果、 (1) 鍼治療と薬物療法の回復の比較では、ENoG値41%以上の群で鍼単独療法群はステロイド経口投与療法群と比べ有意に麻痺の回復が劣った。 (2) ENoG値21%以上の群でステロイド経口投与療法群と鍼併用群群では特に有意差は認められず、鍼を併用しても薬物療法単独群と比べ麻痺の回復は変わらなかった。 (3)...
