Summary We describe the successful use of pharyngeal oximetry with oropharyngeal airway in two patients severe shock whom finger pulse failed. One patient was a 50‐year‐old man septic and other 32‐year‐old woman haemorrhagic shock. In both patients, an paediatric oximeter probe inserted adjacent to tracheal tube. A good waveform obtained oxygen saturation 0–2% lower than arterial samples whereas unobtainable or much saturation. Pharyngeal is feasible more accurate low perfusion states.
