Aneurysm of the left atrial appendage (LAA) is a very infrequent condition. We report case two-year-old child with congenital aneurysm LAA large thrombus in it. He presented an episode seizures left-sided hemiparesis. Diagnosis was based on transthoracic echocardiography and magnetic resonance imaging. The patient successfully treated by surgical resection removal thrombus.

Double orifice mitral valve (DOMV) is a rare congenital anomaly of the apparatus consisting an accessory bridge fibrous tissue, which partially or completely divides into two orifices. It usually occurs as associated lesion. Encountering DOMV isolated finding meriting intervention indeed rare. We, here, report case "complete bridging type DOMV" occurring entity and presenting severe MR requiring surgery.

The clinical detail, echocardiographic features, angiographic evaluation, and surgical repair of double-outlet left atrium (LA) in a 2-year-old boy are discussed. In solitus position normally connected systemic pulmonary venous drainage setting, the only outlet right was secundum atrial septal defect. LA thus received entire return then drained into both ventricles through common atrioventricular valve.
