症例は64歳男性である.1998年に頭部を打撲し慢性硬膜下血腫を来した既往がある.2000年から頭痛,ふらつき,聴力低下が出現し緩徐に増悪した.2011年,他院精査で血性髄液を認め,MRIでT2/T2*強調画像にて脳・脊髄表面に沿った低信号病変を認めた.脳表ヘモジデリン沈着症と診断されるも出血源は不明で,止血剤も無効であった.2015年当科受診時,水平性注視方向性眼振,両側高度感音性難聴,四肢・体幹失調を認め,MRIではCISS法にてTh2~3レベル硬膜前面に欠損を認めた.硬膜欠損に対して硬膜欠損閉鎖術を行ったところ,血性髄液の改善を認め,術後頭痛,失調の改善を認めた.
