Cardiac rhabdomyomas are common in tuberous sclerosis. We report a child who developed rhabdomyoma related arrhythmia refractory to antiarrhythmic drug therapy. Reversion of the atrial ectopic tachycardia was achieved with mammalian target rapamycin pathway ( mTOR ) inhibitor sirolimus. As per our knowledge, this is first time that sirolimus has been successfully used setting.

In most of them diagnosis was made indirectly and only after multiple fetal scans based on findings such as thick dilated left ventricle grossly ascending aorta. We present a patient whom direct visualization at 30th gestation week single scan using recently introduced `cockade sign`. Clinical implications antenatal postnatal treatment outcome also discussed.
