Objective Positron emission tomography/computed tomography ( PET / CT ) has not been well studied as a first‐line test for giant cell arteritis GCA ), due, in part, to historical limitations visualizing the cranial arteries. The Giant Cell Arteritis and Scan GAPS study was therefore carried out assess accuracy of newer generation head, neck, chest determining diagnosis . Methods In cohort, 64 patients with newly suspected underwent time‐of‐flight (1‐mm slice thickness from vertex diaphragm)...
VEXAS is a newly recognised adult-onset autoinflammatory syndrome resulting from somatic mutation in the UBA1 gene. Herein, we present three cases of Sydney, Australia, that capture key clinical features and refractory nature condition. They highlight importance multidisciplinary collaboration for early diagnosis need new therapeutic options.
Objective Giant cell arteritis (GCA) is characterized by cranial ischemia at diagnosis and late aortic structural disease. Repeated combined large‐vessel fluoro‐2‐deoxyglucose (FDG) positron emission tomography (PET)/computed (CT) scans were performed to assess the distribution of vasculitis activity over time relationship with clinical outcomes. Methods Patients eligible if they enrolled in a 64‐patient inception suspected GCA cohort 2016 2017 had positive temporal artery biopsy and/or...
Positron emission tomography/computed tomography (PET/CT) can detect cranial and large vessel inflammation in giant cell arteritis (GCA). We aimed to determine the change significance of vascular activity at diagnosis 6 months.Newly diagnosed GCA patients underwent time-of-flight fluorine-18-fluoro-2-deoxyglucose PET/CT from vertex diaphragm within 72 hours commencing corticosteroids were followed for 12 months. A months scan was performed with inflammatory features on biopsy or CT aortitis....
Abstract Objective First Nations Australians experience a higher burden and severity of Rheumatic Disease with poorer outcomes than the general population. Despite widely acknowledged need to improve health outcomes, there has been minimal research assessing existing models care from perspective in Australia. The objective this study was describe experiences barriers enablers accessing hospital‐based adult Rheumatology service Sydney. Methods A qualitative using semi‐structured interviews...
Abstract Objectives There is uncertainty if varicella zoster virus (VZV) triggers GCA. This based on discordant reports of VZV detection in GCA temporal artery biopsies. We conducted a multimodal evaluation for the inception Giant Cell Arteritis and PET Scan (GAPS) cohort. Methods Consecutive patients who underwent biopsy suspected were clinically reviewed active past infection followed 6 months. Serum was tested IgM IgG. Temporal (TAB) sections stained antigen using Mouse Cocktail Antibody...
Biologics are rapidly emerging as an effective vision saving addition to systemic uveitis therapy. The aim of this multicentre retrospective study is review the outcomes a large group patients treated with adalimumab.A chart refractory non-infectious, active adalimumab was conducted. main outcome measures were ability reduce prednisolone dose, control uveitis, final visual acuity and time treatment failure.Forty-six included in study. most common anatomical phenotype panuveitis (n=17,...

Abstract Aim Diagnosing and monitoring vascular activity in giant cell arteritis (GCA) is difficult due to the paucity of specific serological biomarkers. We assessed utility 8 novel biomarkers an inception cohort newly suspected GCA patients. Method Consecutive patients were enrolled between May 2016 December 2017. Serum was collected within 72 hours commencing corticosteroids at 6 months. It analyzed for levels intra‐cellular adhesion molecule 1, endothelial growth factor (VEGF), pentraxin...
A 55-year-old woman with newly diagnosed Takayasu arteritis was followed for 7 years, during which time she underwent bare metal stenting, drug eluting stenting and coronary bypass grafting critical renal artery stenoses. Interventions were initially successful but restenosis occurred within 24 months all modalities. In contrast, native vessel disease largely stable after the introduction of immunosuppressive therapy. We advocate a conservative revascularization approach in absence end organ...
Abstract Introduction Shear‐wave elastographic ultrasound ( SW ‐ EUS ) assesses the stiffness of human tissues. It is used in liver, thyroid and breast imaging but has not been studied synovium. Soft tissues have a slower shear‐wave velocity SWV than stiff We hypothesised that rheumatoid arthritis RA patients would softer synovium controls this could be quantified with . also assessed whether varied disease activity. Methods Nine were consecutively recruited matched five controls....
Psychosis is a rare manifestation of neuropsychiatric systemic lupus erythematosus (NPSLE). Current guidelines do not make recommendation regarding the use antinuclear antibody (ANA) testing in assessment patients with psychosis. The present study was undertaken to determine prevalence NPSLE psychosis who were positive for ANAs.A retrospective review admitted mental health service 2 metropolitan tertiary referral centers diagnosis and had been tested ANAs conducted. A SLE made when 2019...
There have been reports of COVID-19 vaccination triggering anti-nuclear cytoplasmic antibody (ANCA)-associated vasculitis (AAV), but no robust studies examined the link. This retrospective cohort study assessed impact COVID on rate denovo and relapsed AAV in a Sydney Local Health District from 2018 to 2022. Despite more than 95% population receiving vaccination, case was stable. These findings do not support relationship between AAV.
