目的 探讨剑突下单孔胸腔镜手术(S-UVATS)胸腺扩大切除术治疗胸腺瘤的可行性与临床疗效。 方法 回顾性分析2016年1月—2018年12月徐州市中心医院胸外科采用S-UVATS手术治疗的20例胸腺瘤患者的临床资料。其中男12例,女8例;年龄24~66岁,中位年龄49岁。其中,术前合并重症肌无力的胸腺瘤患者2例。全组患者均在全身麻醉、双腔气管插管下行S-UVATS全胸腺切除并前纵隔脂肪清扫术(胸腺扩大切除术)。观察患者手术时间、出血量、胸腔引流管留置时间与总引流量、手术并发症、术后住院时间,以及术后1~3 d的切口疼痛VAS评分。 结果 本组20例患者,术中有3例患者因无名静脉显示不清晰,中转剑突下三孔胸腔镜手术;无中转开胸。另17例患者顺利完成手术,手术时间50~125 (85.0±20.2) min,术中出血量10~50 (24.5±11.1) mL,术后胸管引流时间1~7(3.0±1.5)d,胸腔引流量50~400...
