Thyromental height test (TMHT) is a recently described anatomical bedside screening tool in predicting difficult laryngoscopy. It has been shown to be more accurate than the modified Mallampati score, thyromental distance (TMD), and sternomental with regard sensitivity positive predictive value (PPV). Airway assessment studies based on anatomic parameters of upper airway are limited subcontinent population. We attempted this study evaluate validate TMHT at 50 mm an Indian population...

More than 10% of adults undergoing colonoscopy have type 2 diabetes (T2D) (1). The use sodium–glucose cotransporter inhibitors (SGLT2i) has increased due to their glycemic control and benefits lowering cardiovascular morbidity mortality as well reducing diabetic nephropathy (2,3). Diabetic ketoacidosis (DKA) is a rare complication associated with SGLT2i. Precipitating factors include fasting, dietary modifications, intercurrent illnesses, surgical stress, insulin insufficiency,...

Summary Whether high‐flow vs. low‐flow nasal oxygen reduces hypoxaemia for sedation during endoscopic retrograde cholangiopancreatography is currently unknown. In this multicentre trial, 132 patients ASA physical status 3 or higher, BMI > 30 kg.m ‐2 with known suspected obstructive sleep apnoea were randomly allocated to up 60 l.min ‐1 at 100% F I O 2 4 . The group also received through an oxygenating mouthguard, totalling 8 Primary outcome was hypoxaemia, defined as S p < 90%...

Abstract Background Atrial fibrillation (AF) is the most common sustained arrhythmia, yet its underlying mechanisms remain poorly understood, leaving treatment options often suboptimal. The autonomic nervous system, and particularly ganglionated plexi (GP) within heart, may hold a critical role in AF pathogenesis. Emerging evidence points to structural functional remodeling GP neurons AF, but physiology of human still inadequately described. Transcutaneous vagus nerve stimulation (tVNS) has...

Abstract Background Various airway techniques have been employed for endoscopic procedures, with an aim to optimise patient outcomes by improving control and preventing hypoxia whilst avoiding the need intubation. The LMA® Gastro™ Airway, a novel dual channel supraglottic technique, has described as such device. Its utility alongside sedation low flow nasal cannula general anaesthesia (GA) intubation retrograde cholangiopancreatography (ERCP) procedures was evaluated. Methods Details of all...

A prospective, randomized double-blind trial was performed to compare the effects of 4% endotracheal tube cuff lignocaine and 1.5 mg/kg intravenous (IV) on coughing hemodynamics during extubation in patients undergoing elective craniotomies supine position. Forty-one received into after intubation (ETT group) 41 IV at before (IV group). Coughing assessed by a scale 3 time extubation. Hemodynamic parameters recorded 1-minute interval for 5 minutes were compared with baseline values skin...

Summary Following abdominal surgery, the provision of postoperative analgesia with local anaesthetic infusion through both transmuscular quadratus lumborum block and pre‐peritoneal catheter have been described. This study compared these two methods following laparotomy. Eighty‐two patients 18–85 years age scheduled to undergo elective surgery were randomly allocated receive either or block. In group, an 18‐gauge Tuohy needle was passed muscle under ultrasound guidance reach its anterior...

Abstract Background Endoscopic retrograde cholangiopancreatography (ERCP) is an increasingly common intervention in the treatment of pancreaticobiliary disorders. Patients are often elderly with complex co-morbidities. While monitored anaesthesia care sedation commonly used for most cases, few would require general endotracheal tube. Both low-flow and high-flow nasal cannulas (HFNC) established ways delivering supplemental oxygen, but it unclear whether one technique better than other. HFNC...
