We describe a 78-year-old woman with large ascending aortic pseudoaneurysm who underwent thoracic endovascular repair under intraoperative image fusion guidance and real-time transcranial Doppler (TCD) monitoring. TCD monitoring revealed total of 419 microembolic signals throughout the procedure, majority occurring as first stent graft crossed aorta. Two days later, she type IA endoleak. highlight role in enabling successful an elderly identifying procedural areas improvement to minimize stroke risk.
