BackgroundComplex or malignant left-sided heart tumors present a challenge for resection and reconstruction. Cardiac autotransplantation was developed to address this but is complex technique. Partial simplify the procedure. This study evaluates outcomes of partial cardiac compared with those full autotransplantation.MethodsWe analyzed our prospectively collected tumor database cases between 1998 2022 requiring autotransplantation. The primary end points were morbidities including bleeding,...
This case reviews a challenging but successful transcatheter coil embolization of large congenital coronary artery fistula (CAF) causing significant left-to-right shunt.A 51-year-old female with no prior history presented symptoms dyspnoea and chest discomfort. Extensive evaluation revealed CAF between tortuous right (RCA) the sinus (CS) composed three aneurysmal pseudochambers. Closure RCA-CS was attempted through deployment into neck. However, due to brisk flow fistula, both coils...
We describe a 78-year-old woman with large ascending aortic pseudoaneurysm who underwent thoracic endovascular repair under intraoperative image fusion guidance and real-time transcranial Doppler (TCD) monitoring. TCD monitoring revealed total of 419 microembolic signals throughout the procedure, majority occurring as first stent graft crossed aorta. Two days later, she type IA endoleak. highlight role in enabling successful an elderly identifying procedural areas improvement to minimize stroke risk.
Papillary muscle rupture due to infective endocarditis is a rare event and proper management of this condition has not been described in the literature. Our case aims shed light on treatment strategies for these patients using current guidelines.
A 60-year-old man was about to undergo minimally invasive aortic valve replacement when transesophageal echocardiography revealed an intracardiac mass on the left atrial free wall. Multimodal images from 5 months earlier had shown no mass. We converted procedure open surgery. The excised resembled a cardiac myxoma but determined be papillary fibroelastoma. This case illustrates that fibroelastomas can form and grow rapidly, warranting alertness for their unexpected discovery before during...
Cardiorenal syndrome is a complex characterized by dysfunction of the heart and kidneys in an interdependent fashion further divided into different subtypes based on primary organ dysfunction. Simultaneous Heart-Kidney transplantation treatment choice for end-stage irreversible both organs, however it may be avoided with determination cardiorenal subtype management This article discusses types syndrome, indications concerns regarding use simultaneous heart-kidney transplantation, outlines...
Cardiac tumors in adults are exceedingly rare and usually benign. We describe a 29-year-old man with previous diagnosis of interventricular septal hypertrophy who presented increasing severity dyspnea fatigue. Work-up revealed 4.9 × 3.7 cm mass at the base septum. Biopsy benign cardiac hamartoma atypically located right ventricle, was resected via ventriculotomy.
Four patients with pulmonary valve (PV) disease and patent foramen ovale (PFO) presented dyspnea on exertion. Work-up revealed hypoxemia secondary to right-to-left intracardiac shunt. We demonstrate that correction of the primary culprit right heart overload lesion via PV replacement enabled safe PFO repair resolution hypoxemia.
A 67-year-old woman with prior transcatheter aortic valve replacement presented worsening dyspnea. Imaging revealed thrombosis and stenosis. Despite oral anticoagulation, she progressively deteriorated developed cardiogenic shock. We highlight the Heart Team's role in treating this unusual late thrombosis. (Level of Difficulty: Intermediate.).
We describe an 81-year-old man with end-stage renal disease and central venous occlusion who was referred for dialysis access creation. This case illustrates a novel percutaneous image fusion-guided recanalization of occluded right subclavian vein brachiocephalic stent in patient limited remaining sites. (Level Difficulty: Advanced.).
Central MessageExtra-anatomic procedures in infancy and childhood lead to late sequalae adulthood, necessitating lifelong surveillance interval imaging patients with complex congenital heart disease. We describe a 31-year-old man born type B interrupted aortic arch an aberrant origin of the right subclavian artery from common trunk, malalignment ventricular septal defect, patent ductus arteriosus, bicuspid valve. The institutional review board or equivalent ethics committee authors'...
We describe an 88-year-old woman who experienced annular rupture during transcatheter aortic valve replacement despite preventative measures. She underwent Y incision and rectangular patch for the double purpose of repairing enlarging root. highlight heart team's role in confronting this potentially catastrophic complication. (Level Difficulty: Advanced.).
Growth of a spinal arachnoid cyst into the posterior mediastinum is rare phenomenon not described in literature. We present case 30-year-old patient with chest pain, cough, fatigue, and occasional dysphagia who was found to have an 8 × 6.5 9.5-cm located on imaging. The taken operating room, where clear fluid positive for β2-microglobulin consistent cerebrospinal encountered. Dural communication canal identified, confirming diagnosis cyst. This report aims describe this thoracic disease...
We describe a 60-year-old man with history of hypertension who presented to an outside emergency department chest pain and left lower extremity numbness weakness. Computed tomography (CT) revealed Stanford type A aortic dissection (TAAD), he was transferred our institution for emergent open surgical repair. Review the CT showed no flap in ascending aorta complex proximal descending thoracic consistent intimal transection at sinotubular junction intimointimal intussusception. This case...
We describe a giant right coronary artery (RCA) to sinus (CS) fistula in 59-year-old woman who presented our institution with heart enlargement.Investigation revealed an ectatic 22mm RCA draining into enlarged CS, and Qp:Qs of 2.0. proceeded surgery. After initiating cardiopulmonary bypass electrical arrest, the distal was opened. An opening CS confirmed closed bovine pericardium patch. The origin triply ligated. Anastomoses saphenous vein graft posterior descending ventricular marginal were...
Abstract Background: Papillary muscle rupture due to infective endocarditis is a rare event and proper management of this condition has not been described in the literature. Our case aims shed light on treatment strategies for these patients using current guidelines. Case Presentation: This presents 58-year-old male with acute heart failure secondary papillary rupture. He underwent an en bloc resection his mitral valve bioprosthetic replacement. Specimen pathology later showed necrotic...
We describe a 39-year-old man referred for surgical aortic valve replacement severe symptomatic stenosis. Intraoperative inspection was unexpectedly consistent with marantic endocarditis. Pathology confirmed nonbacterial thrombotic present high-resolution intraoperative, diagnostic, and pathology images of endocarditis in patient antiphospholipid syndrome atypical presentation.
We sought to examine the efficacy and safety of adding fibrinogen-guided low-dose multi-day Alteplase™ tissue plasminogen activator (tPA) in management intravascular hemolysis (IVH) patients with HeartMate II (HM-II) continuous flow (CF) left ventricular assist device (LVAD) who failed achieve IVH resolution conventional augmented anticoagulation (AAC).IVH LVAD is often treated AAC, failing which pump exchange considered. hypothesized that a trial tPA after AAC therapy could resolve prevent...
We describe a 64-year-old woman with subclavian pseudoaneurysm after aortic coarctation repair, treated using hybrid approach involving true three-dimensional analysis and image fusion-guided placement of thoracic endovascular repair stents. This case illustrates the potential complications need for lifelong surveillance in these patients. (Level Difficulty: Advanced.)
