高齢者に対する硬膜外ブロックでは血圧が下降することが多く,しかもブピバカインを使用した場合はこれの心筋抑制作用のため適当な局所麻酔薬とはいえない.今回著者らは2% (0.1ml/kg),4%および6% (0.05ml/kg)のリドカインを高齢者12例の胸部硬膜外ブロックに用いたところ,2%では経過中有意に収縮期圧が下降したが,6%では安定した結果が得られた.一方,6%リドカインの血中濃度の推移を調べるため予定手術患者63例において頸部,胸部,腰部にそれぞれ0.1ml/kgを注入した結果,頸部では有意にCmax (7.6±1.3μg/ml)が高く,腰部では頸部に対し有意にTmax (8.8±2.0min)が速かった.また頸部では腰部に対し有意に低いKaを示した.以上より高齢者で少量の6%リドカイン(0.1ml/kg以下)を使用することは安全でかつ有効な方法と考えられた.
