Pregnancy increases metabolic demand for insulin and may lead to the exhaustion of intraportally transplanted islets post-gestational hyperglycemia. To prevent these complications, we implemented preemptive supplementation during two subsequent pregnancies in an insulin-independent islet transplant recipient.
