瞳孔大小及光反射的檢查,在頭部外傷的病患照護上是必要的神經學評估工具。頭部外傷的病患若瞳孔放大,可能表示顱內出血的情況惡化、腦壓上升、腦疝脫,或是發生動眼神經壓迫性病灶。我們在此提出一個頭部外傷的案例,其最初格拉斯哥昏迷指數為E3M6V3且瞳孔對稱。其昏迷指數在短暫觀察後回復至15分。病患在急診接受右眼角附近撕裂傷的縫合後轉至神經外科病房,於神經外科病房出現右側瞳孔放大且無光反射的現象,此現象在12小時後自行緩解,且未伴隨其他神經學症狀。我們發現此案例單側瞳孔放大是歸因於右眼角撕裂傷縫合時,使用局部注射麻醉藥Lidocaine所引起,並針對相關機制及文獻提出討論。
