外傷性鎖骨下動脈損傷は第1肋骨骨折に合併し,受傷直後に発症することが多い。今回,受傷36時間後に鎖骨下動脈損傷を発症し,血管内治療で救命し得た症例を経験した。症例は60歳代の女性。交通事故のため救急搬送された。右多発肋骨骨折,右血気胸,左第1肋骨骨折,骨盤骨折を認めた。左第1肋骨骨折端は鋭利な斜骨折をきたしていた。右血気胸に胸腔ドレーン挿入,骨盤骨折に創外固定を行い,入院した。受傷36時間後に誘因なく突然,左鎖骨部の疼痛と腫脹を認め,ショック,呼吸不全となった。補液により循環動態は安定し,造影CTで左血胸と第1肋骨骨折部の近傍に左鎖骨下動脈損傷を認めた。左血胸には胸腔ドレーンを留置し,鎖骨下動脈損傷には血管内ステントグラフトを留置した。その後は状態が安定し,第14病日にはICUを退室した。第1肋骨の鋭利な骨折端が鎖骨下動脈の近傍にある場合は,遅発性鎖骨下動脈損傷が起こる可能性があり,注意して経過をみる必要がある。
