IgG4関連疾患(IgG4-RD)は潜在性に腎不全を引き起こす.血液透析患者における高IgG4血症,IgG4-RDの頻度を調査することを目的に,対象者227例において血清IgG4値を測定し,平均血清IgG4値が34 mg/dL(範囲3~329)で2.2%にIgG4≥135 mg/dL,5.3%にIgG4≥105 mg/dL(基準値上限)を認めた.年齢,性別,生活習慣,悪性腫瘍などの既往・併存症,各血液検査値について,線型回帰分析により血清IgG4値との関連を探索したところ,男性,低CH50血症,総Cho低値,抗核抗体陽性,非高血圧,喫煙歴が関連していた.高値例12例に対して診療録を精査し,IgG4-RDによる腎不全症例1例を認めた.透析患者における高IgG4血症は男性,喫煙,免疫異常を示唆する所見などと関連し,高値例の一部に潜在性患者も存在する.腎不全の原因としてIgG4-RDも考慮する必要がある.
