肝癌診療ガイドライン（2021年版）では，肝細胞癌の腹膜転移に対しては分子標的薬が推奨されている．結腸直腸癌の腹膜転移に関しては，腫瘍減量手術・術中腹腔内温熱化学療法はNCCNガイドラインでも考慮されており，肝細胞癌の腹膜播種に対してもその有効性が期待される．症例は70歳代男性で，肝細胞癌破裂に対して肝切除術を施行し，術後10カ月で腹膜転移再発と診断した．レンバチニブによる化学療法を開始したが有害事象が出現し，化学療法6カ月施行後も腫瘍進行を認めなかったことから結腸切除を含む部分腹膜切除術，術中腹腔内温熱化学療法を施行した．手術時，横隔膜下，モリソン窩，大網に結節性病変（腹膜播種係数＝6）を認めた．手術は減量切除スコア＝0を達成した．術後合併症も無く術後12日目に退院となった．術後補助化学療法は施行せず，食事摂取と筋肉量も改善した．術後24カ月経過した現在も無再発生存中である．

びまん性悪性腹膜中皮腫は本邦では切除不能と判断し，悪性胸膜中皮腫に準じた化学療法を施行することが多い．一方海外では，腹腔内に限局し末期まで遠隔転移することが少なく，腹膜切除が有効であると報告されている．症例は64歳の男性で，腹部膨満，下腿浮腫を認めた．造影CTで多量腹水，腹膜肥厚，大網肥厚・結節が指摘され，腹水細胞診で悪性中皮腫と診断された．全腹膜切除術（直腸，回盲部，脾臓，胆囊合併切除），回腸人工肛門造設術，腹腔内化学療法を施行した．手術時，腹膜，小腸間膜，結腸間膜，大網，小網にびまん性の小隆起病変（腹膜播種係数＝26）を認めた．手術は減量切除スコア＝0を達成した．術後は下痢，胃内容停滞を認めたが，第45病日で退院となった．術後補助化学療法は施行せず，その後食事摂取と筋肉量も改善した．1年10か月後に再発を認め，再発腫瘍切除および術中温熱化学療法を施行し，経過観察中である．
出血性肝囊胞破裂についての報告は極めて少ない．91歳女性の肝囊胞破裂による出血性ショックに対して緊急手術を行った1例を報告する．腎盂腎炎，敗血症性ショックと診断され前医で加療されていた．来院当日，急激な右下腹部痛を自覚しプレショック状態となり当院に転送された．造影CTで肝後区域に高吸収域を伴う囊胞性病変を認めた．周囲に高CT値の腹水を認め，肝囊胞性病変の出血性破裂によるショックと診断した．Extravasationを伴わなかったため経カテーテル的止血術ではなく緊急開腹術を選択した．プリングル下に肝右葉を授動し，肝囊胞壁に到達した．肝囊胞内からの出血を確認し，肝囊胞出血性破裂と診断した．囊胞壁を切除し肝囊胞内に露出したグリソンからの出血部位を縫合した．ダグラス窩の腹水が混濁していたため，肝囊胞感染が契機に出血し，破裂したと考えた．91歳と高齢であったが術後14病日に転院した．
