To study the association between time to antibiotic administration and survival in patients with severe sepsis or septic shock whom early goal-directed therapy was initiated emergency department.Single-center cohort study. The department of an academic tertiary care center from 2005 through 2006.Two hundred sixty-one undergoing therapy.None.Effects different cutoffs triage administration, qualification for appropriate on in-hospital mortality were examined. The mean age 261 59 +/- 16 yrs; 41%...
This study investigates the clinical and pathologic findings associated with 173 primary brain tumors in our hospital population of dogs that presented between years 1986 2002. Of tumors, 78 (45%) were meningiomas, 29 (17%) astrocytomas, 25 (14%) oligodendrogliomas, 12 (7%) choroid plexus 7 (4%) central nervous system lymphomas. Smaller numbers glioblastomas ( n = 5), primitive neuroectodermal histiocytic sarcomas vascular hamartomas 4), unclassified gliomas 3) identified. One dog had both a...
Objective: The objective of this study was to determine serum (perfluorooctanoate [PFOA]) in residents near a fluoropolymer production facility: the contributions from air, water, and occupational exposures, personal dietary habits, relationships age gender. Methods: authors conducted questionnaire PFOA measurements stratified random sample volunteers residing locations with same residential water supply but higher lower potential air exposure. Results: Serum (PFOA) greatly exceeded general...
The objective was to study the association between factors related emergency department (ED) crowding and patient satisfaction. The authors performed a retrospective cohort of all patients admitted through ED who completed Press-Ganey satisfaction surveys over 2-year period at single academic center. Ordinal binary logistic regression used validated (such as hallway placement, waiting times, boarding times) with both care assessment overall hospitalization.A total 1,501 hospitalizations for...
Oligoanalgesia for acute abdominal pain historically has been attributed to the provider's fear of masking serious underlying pathology. The authors assessed whether a gender disparity exists in administration analgesia pain. This was prospective cohort study consecutive nonpregnant adults with nontraumatic less than 72 hours' duration who presented an urban emergency department (ED) from April 5, 2004, January 4, 2005. main outcome measures were and time analgesic treatment. Standard...
While emergency department (ED) crowding is a worldwide problem, few studies have demonstrated associations between and outcomes. The authors examined whether ED was associated with adverse cardiovascular outcomes in patients chest pain syndromes (chest or related complaints of possible cardiac origin).A retrospective analysis performed for >or=30 years age syndrome admitted to tertiary care academic hospital from 1999 through 2006. compared rates inpatient triage discharge, defined as...
Abstract. Objective: Research defining optimal methods of intubation has been limited by the lack a validated outcome measure to assess airway visualization. The objective this study was develop reliable scale for assessment visualization during endotracheal intubation. Methods: This prospective performed intra‐ and interphysician reliabilities emergency physicians (EPs) estimating percentage glottic opening (POGO) that is visualized direct laryngoscopy. Using video images laryngeal views...
Patients presenting with chest pain or related symptoms suggestive of myocardial ischemia, without ST-segment elevation (NSTE) on their electrocardiograms, often present a diagnostic challenge in the emergency department (ED). Prompt and accurate risk stratification to identify those patients NSTE who are at highest for adverse events is essential, however, optimal management. Although validated used frequently already enrolled acute coronary syndrome trials, Thrombolysis Myocardial...
The purpose of this study was to determine the frequency different tumor types within a large cohort cats with intracranial neoplasia and attempt correlate signalment, size location, survival time for each tumor. Medical records 160 confirmed evaluated between 1985 2001 were reviewed. Parameters included age, sex, breed, FeLV/FIV status, clinical signs, duration number tumors, location(s), imaging results, treatment, times, histopathologic diagnosis. Most older (11.3 +/- 3.8 years). Primary...
To determine postoperative wound infection rates in dogs and cats for various contamination categories to identify factors that influence rates.Epidemiologic study.1,574 wounds 1,255 cats.Information recorded included signalment, nutritional status, surgery duration, surgical procedures, classification, interval from clipping until surgery, blood pressure values, active at a distant site, endocrinopathy, administration of immunosuppressive medications or antibiotics. Relative risk, 95%...
Objective: The objective of this study was to determine whether certain biomarkers toxicity and/or a past diagnosis liver or thyroid disease were associated with serum perfluorooctanoate concentrations (PFOA) in community longstanding environmental exposure PFOA. Methods: Serum (PFOA), hematologic and biochemical biomarkers, questionnaire administered 371 residents selected by stratified random sampling lottery among volunteers. Median PFOA 354 ng/mL (interquartile range, 181–571 ng/mL)....
Methicillin resistance rates of 41% for Staphylococcus aureus, 16% S. intermedius, and 40% schleiferi have recently been reported in canine patients. These were deemed to be reflective referral clinician-selection biases, which would imply significantly lower methicillin-resistant staphylococcal carriage less-biased populations. In this study, swabs bacterial culture collected from five cutaneous sites on each 50 healthy dogs 59 with inflammatory skin disease determine prevalence relative...
Resiniferatoxin is a potent capsaicin analog. Intrathecal administration leads to selective, prolonged opening of the transient receptor potential V1 ion channel, which localized mainly C-fiber primary afferent nociceptive sensory neurons. Following work in laboratory animals, authors explored use intrathecal resiniferatoxin control spontaneous bone cancer pain companion (pet) dogs.Normal canine population: Behavioral testing was performed establish baseline paw withdrawal latency;...
Abstract Objective —To evaluate the effects of diet restriction on development radiographic evidence hip joint osteoarthritis in dogs. Design —Longitudinal cohort study. Animals —48 Labrador Retrievers from 7 litters. Procedures —Forty-eight 6-week-old puppies litters were paired with littermates by sex and weight, each pairmate was randomly assigned to 1 2 groups 24 dogs each. Starting at 8 weeks age, group fed ad libitum (control fed) other 25% less (restricted same for life a pairwise...
In this study, we have characterized the response of five sequential segments urinary bladder to cholinergic, adrenergic, and purinergic agonists. The results indicate that an abrupt alteration in pharmacological between body base occurs at level ureteral orifices. responds preferentially urecholine (muscarinic cholinergic), isoproterenol (beta adrenergic) ATP (purinergic), whereas perferentially methoxamine alpha adrenergic). parallel studies, density both muscarinic cholinergic receptors...
