【目的】術後管理目的にICUへ入室した患者を対象にし,抜管直前の呼吸機能検査における再挿管の関連因子を検討した。【方法】2016年6月から2018年7月の間に術後管理目的で人工呼吸器装着のままICUへ入室した症例のうち,抜管直前に呼吸機能検査を行えた症例を対象とした。対象者を抜管後72時間以内の再挿管の有無で再挿管群と非再挿管群に分けた。 APACHE Ⅱscore,性別,術式にて1:2でマッチングし,背景因子と抜管直前の呼吸機能検査値を比較検討した。【結果】全324例中再挿管群は9例(2.8%)であった。背景因子はマッチングした非再挿管群の18例との間に差を認めなかった。呼吸機能検査値では最大呼気圧(maximal expiratory pressure, MEP)のみ再挿管群で有意に低値であった。再挿管理由の半数以上が痰の喀出困難であった。【結論】MEPが低値であると再挿管に至る可能性が高いことが示唆された。
