We observed a marked prolongation of the transcranially evoked silent period during continuous intrathecal administration high doses gamma-aminobutyric acid (GABA)B receptor agonist baclofen in patient with generalized dystonia. Size motor potentials and central conduction time remained unchanged administration. The selective high-dose therapy supports notion that GABAB-ergic intracortical interneurons play part generation period. © 1998 John Wiley & Sons, Inc. Muscle Nerve 21:1209–1212, 1998.
Lesions of the central nervous system often result in an upper motor neuron syndrome including spasticity, paresis with pyramidal signs, and painful spasms. Pharmacological treatment oral antispasticity drugs is frequently associated systemic side effects which limit their clinical use. Botulinum Toxin A (BtxA) injected spastic muscles has been shown to be effective reducing muscle tone, but only few studies have reported pain relief as additional benefit. Therefore, we investigated local...
We report on eight patients with stiff-man syndrome (SMS) or its "plus" variant, progressive encephalomyelopathy rigidity and myoclonus (PERM) receiving intrathecal baclofen via pump. In six of the patients, follow-ups continued for approximately 2.5 to 6.5 years after pump implantation. Intrathecal was an effective last-resort alternative who responded poorly did not tolerate oral antispasticity medications. General mobility increased, spasms were reduced; however, no complete remissions...
Robotic therapy and serious gaming support motor learning in neurorehabilitation. Traditional monitor-based outputs cannot adequately represent the third dimension, whereas virtual reality headsets lack connection to real world. The use of Augmented Reality (AR) techniques could potentially overcome these issues. objective this study was thus evaluate usability, feasibility functionality a novel arm rehabilitation device for neurorehabilitation (RobExReha system) based on robotic (LBR iiwa,...
Abstract Intrathecal baclofen dramatically improves severe spastic syndromes. This improvement is likely related to reduced excitability of alphamotoneurons. To investigate the influence upon alphamotoneuron, we analyzed F‐waves before and after intrathecal bolus injection (usually 50 μg) as well administration different, constantly delivered doses (60–200 μg/day). decreased maximum F‐wave amplitude (F p ) from an initial value 9% M (M max (=F/M‐ratio) 2.4% 130–180 min, mean 60% within 150...
To investigate whether the dose of intrathecal baclofen necessary for a sufficient reduction muscle tone and spasms changes during treatment severe spasticity.A group 27 patients received 61 (SD 18) months.Spasticity remained absent or strongly reduced after stopping infusion in seven patients. The could be to 40% that which was originally 10 same increased slightly Possible reasons continuing spasticity terminating long term some be: lasting morphological spinal cord neurons by second...

Continuous intrathecal baclofen infusion (CIBI) is a highly effective treatment for severe spasticity, especially of spinal origin.1-2 The beneficial effect CIBI on spasticity related to its gamma-aminobutyric acid (GABA)B-ergic action interneurons, resulting in profound depression disinhibited mono- and polysynaptic reflexes.1-3 Although the effects reflexes have been studied detail,2-4 corticofugal projections motor neurons not characterized.
Epileptic discharges can be elicited through different stimuli. In conscious patients hyperventi-lation and intermittent photic stimulation is used, in comatose other modalities such as reagebi-lity to auditory, tactile visual stimuli are tested.
Abstract Introduction The WHO estimates that each year 5 million people are left permanently disabled after stroke. Adjuvant treatments to promote the effects of rehabilitation urgently needed. Cortical excitability and neuroplasticity can be enhanced by non-invasive brain stimulation but evidence from sufficiently powered, randomized controlled multi-center clinical trials is absent. Methods Neuroregeneration transcranial direct current (tDCS) in stroke (NETS) tested efficacy safety anodal...
