Clinicians and researchers have shown increasing interest in frailty. Yet, there is still considerable uncertainty regarding the concept its definition. In this article, we present perspectives on key issues controversies discussed by scientists from 13 different countries, representing a diverse range of disciplines, at 2006 Second International Working Meeting Frailty Aging. The following fundamental questions are discussed: What distinction, if any, between frailty aging? relationship...
Background. Frailty and fitness are important attributes of older persons, but population samples their prevalence, attributes, outcomes limited. Methods. The authors report data from the community-dwelling sample (n = 9008) Canadian Study Health Aging, a representative, 5-year prospective cohort study. Fitness frailty were determined by self-reported exercise function level testing cognition. Results. Among elderly population, 171 per 1000 very fit 12 frail. increased with age, so that age...
Dementia shortens life expectancy; estimates of median survival after the onset dementia have ranged from 5 to 9.3 years. Previous studies people with existing dementia, however, may underestimated deleterious effects on by failing consider persons rapidly progressive illness who died before they could be included in a study (referred as length bias).
Recent reports suggest a possibly protective effect for statins in patients with Alzheimer disease. This association could be due to indication bias, i.e., people who elect take lipid-lowering agents (LLAs) may healthier than those do not, so that it these other health factors explain their lower risk of dementia. To examine the between use LLAs and dementia, adjusting markers health, investigate associated LLA use.A cohort study case-control dementia relation use, secondary analysis Canadian...
The Canadian Neurological Scale (CNS) was designed to monitor mentation and motor functions in stroke patients. We assessed its validity reliability on a group of 157 patients with diagnosis acute cerebrovascular accident. determined by (1) correlating scale items total score the standard neurologic examination; (2) exploring scale9s predictive power different end points at 6 months—the initial CNS significant predictor outcome; (3) showing that had higher correlation coefficients...
Idiopathic rapid eye movement (REM) sleep behavior disorder (RBD) is a parasomnia that an important risk factor for Parkinson's disease (PD) and Lewy body dementia. Its prevalence unknown. One barrier to determining current screening tools are too long large-scale epidemiologic surveys. Therefore, we designed the REM Sleep Behavior Disorder Single-Question Screen (RBD1Q), question dream enactment with simple yes/no response.Four hundred eighty-four sleep-clinic-based participants (242...
Acute central nervous system dysfunction resulting in coma can be measured simply and reliably by the Glasgow scale. However, when injury does not impair consciousness patient has aphasia, no comparable scale exists. A complementary to assess conscious aphasic patients is proposed. Preliminary validation been carried out acute stroke patients, who commonly suffer neurological deficits without loss of consciousness. simple standardized aids monitoring status, may help assessment prognosis therapy.
A recent paper by Bialystok et al in Neuropsychologia (vol. 45, pgs. 459 to 464) suggested that early bilingualism produced a statistically significant 4.1-year delay onset of memory loss symptoms older individuals with Alzheimer disease, possibly reflecting an increase the cognitive reserve these individuals. That study focused on multilingual elderly patients whom 90% were immigrants. Our clinic, Montreal Canada, has advantage having large set who are either immigrants or nonimmigrants but...
To evaluate the direct and indirect influences of physical comorbidity, symptoms depression anxiety, fatigue, disability on health-related quality life (HRQoL) in persons with multiple sclerosis (MS).A large (n = 949) sample adults MS was recruited from 4 Canadian clinics. HRQoL assessed using patient-reported Health Utilities Index Mark 3. Expanded Disability Status Scale scores, depression, fatigue were evaluated as predictors a cross-sectional path analysis. All significantly associated...
Drug therapies for Alzheimer’s disease (AD) have been evaluated in clinical trials over the past 2 decades. Systematic reviews of AD drug can shed more light on efficacy pharmaceutical interventions. The modified Jadad scale be used to assess quality trial reports that are candidates inclusion these systematic reviews. interrater reliability was examined during such a review. Three blinded reviewers rated 42 reports: intraclass correlation coefficient 0.90. appears useful tool research...
To determine whether prevalent delirium is an independent predictor of mortality in older patients seen emergency departments (EDs) and discharged home without admission.Prospective study with 18 months follow-up.EDs two Montreal hospitals. From a cohort prognosis for (107 delirious 161 nondelirious subjects), 30 77 subjects aged 66 who were admission identified.Detailed interviews their proxies review medical charts performed at enrollment. Trained lay interviewers determined status using...
In Canada, half the people with dementia live in institutions. Factors associated institutionalization should be identified goal of implementing strategies not only to permit those stay their homes as long is feasible but also ensure that steps are taken for timely when appropriate.Informal caregivers 326 individuals living community were and interviewed part Canadian Study Health Aging (CSHA). These subjects contacted again 2.5 5 years after baseline interview collect information on status...
Little is known about progression, short of dementia, in vascular cognitive impairment. In the Canadian Study Health and Aging, 149 participants (79.3 +/- 6.7 years; 61% women) were found to have impairment, no dementia (CIND). After 5 years, 77 (52%) had died 58 (46%) developed dementia. Women at greater risk (OR 2.1, 1.0 4.5). Of 32 alive without cognition deteriorated seven improved four. Half those with CIND within suggesting a target for preventive interventions.
Purpose : To investigate the measurement properties of Quebec User Evaluation Satisfaction with assistive Technology (QUEST 2.0) respect to test-retest stability, alternate form reliability, construct validity and applicability. Method Data on satisfaction quality life impacts mobility devices were obtained from 81 community-based adults Multiple Sclerosis, using QUEST 2.0 Psychosocial Impact Assistive Devices Scale (PIADS). Subjects assigned four groups a second was administered one week...
Abstract Decreases in brain N ‐acetylasparatate are associated with neuronal loss or dysfunction. We report a longitudinal study which changes the ‐acetylaspartate to creatine resonance intensity ratio measured by proton magnetic spectroscopy were used follow progression of pathology 7 patients multiple sclerosis over an 18‐month period. Four had history recurrent relapses and 3 secondary progressive course. All clinical imaging evidence persistent neurological abnormalaties. At 6‐month...
This study aimed to develop and validate a brief tool for physician use improve suspicion about the presence or absence of elder abuse. A literature review on abuse, obstacles its identification, limitations detection tools, characteristics screeners employed by physicians were used generate abuse questions critique 31 doctors, nurses, social workers in focus groups. Six resulting became Elder Abuse Suspicion Index (EASI) administered 104 family doctors 953 cognitively intact seniors...
