背景：家族性地中海熱（FMF）は不明熱の原因としては稀であるが，近年本邦でも疾患認知が高まり，診断数が増加してきている．著者らは「短期間で終息する発熱エピソードが反復する」という発熱パターンに注目することによって，一般内科外来で多くのFMFを診断してきた．当科FMF30例の臨床データを解析することにより，診断のための注意点について論ずることを本調査研究の目的とした．対象：2012年9月02015年8月までの3年間に当科でFMFとして診療された全患者を対象とした．Tel-Hashomer基準を満たさないものは除外した．結果：対象となったのは38例で，このうちTel-Hashomer基準を満たしていたのは30例だった．14例でMEFV遺伝子変異が見いだされ，変異の有る例が少ない傾向にあった．平均発症年齢は27.8歳と高かったが，典型発作を有する例は17例（56.7％），コルヒチン抵抗性例は3例（10.7％）と既報とほぼ同等だった．結論：FMFは外来に多く潜在し得る．発熱するが数日で自然軽快し，それを周期性に反復する病像に注目すべきである．
