Pulmonary artery aneurysm is rare condition. Different etiologies have been reviewed, but idiopathic lesions without other symptoms are seldom reported. We report the case of a 73-year-old female with main pulmonary 54 mm. There was no accompanying abnormality. Because patient old, had severe and there were signs aneurysmic compression or shunt, initial considerations surgical intervention turned down. This highlights conservative management such affected patients, regular follow-up.
