Illicit opioids, consisting largely of fentanyl, novel synthetic and adulterants, are the primary cause drug overdose fatality in United States. Xylazine, an alpha-2 adrenergic agonist veterinary tranquilizer, is being increasingly detected among decedents following illicit opioid overdose. Clinical outcomes non-fatal involving xylazine unexplored. Therefore, emergency department patients with overdose, we evaluated clinical outcome differences for without exposures.
Induction of buprenorphine, an evidence-based treatment for opioid use disorder (OUD), has been reported to be difficult people with heavy fentanyl, the most prevalent in many areas country. In this population, precipitated withdrawal (POW) may occur even after individuals have completed a period abstinence prior induction. Our objective was study potential associations between buprenorphine induction, and POW, using social media data. This is mixed methods data from seven opioid-related...
Objectives Xylazine is an α 2 -agonist increasingly prevalent in the illicit drug supply. Our objectives were to curate information about xylazine through social media from people who use drugs (PWUDs). Specifically, we sought answer following: (1) What are demographics of Reddit subscribers reporting exposure xylazine? (2) Is a desired additive? And (3) what adverse effects PWUDs experiencing? Methods Natural language processing (NLP) was used identify mentions “xylazine” posts by also...
Sodium nitrite is a powerful oxidizing agent that causes hypotension and limits oxygen transport delivery in the body through formation of methemoglobin. Clinical manifestations can include cyanosis, hypoxia, altered consciousness, dysrhythmias, death. The majority reports on sodium poisonings have been result unintentional exposures. We report case an intentional fatal overdose nitrite. A 17-year-old female reportedly drank approximately one tablespoon self-harm attempt. patient was...
Introduction Medications such as buprenorphine and methadone are effective for treating opioid use disorder (OUD), but many patients face barriers related to treatment access. We analyzed two sources of data—social media published literature—to categorize quantify barriers. Methods In this mixed methods study, we social (Reddit) posts from three OUD-related forums (subreddits): r/suboxone, r/Methadone, r/naltrexone. applied natural language processing identify relevant barriers, categorized...
In June 2022, the United States Supreme Court decision Dobbs v. Jackson Women's Health Organization overturned Roe Wade, removing almost 50 years of precedent and enabling imposition a wide range state-level restrictions on abortion access. Historical data from internationally demonstrate that removal safe options will increase complications health risks to pregnant patients. Because emergency department is critical access point for reproductive care, clinicians must be prepared policy,...
Background: The safety and efficacy of high dose buprenorphine (BUP) induction in fentanyl positive emergency department patients (high BUP) study includes two clinical trials funded through the helping to end addiction long-termSM initiative. tests whether initiation continuation BUP at higher doses over a shorter time period than currently recommended is safe, tolerable effective. Methods: Trial 1 head-to-head comparison safety, tolerability feasibility (32 mg) (24 as compared standard (12...
Background According to the latest medical evidence, Methadone and buprenorphine-naloxone (Suboxone®) are effective treatments for opioid use disorder (OUD). While evidence basis of these medications is favorable, less known about perceptions general public them. Objective This study aimed Twitter assess methadone buprenorphine-naloxone, compare their discussion contents based on themes/topics, subthemes, sentiment. Methods We conducted a descriptive analysis small automatic large volume...
Biomedical research involving social media data is gradually moving from population-level to targeted, cohort-level analysis. Though crucial for biomedical studies, user's demographic information (eg, gender) often not explicitly known profiles. Here, we present an automatic gender classification system and illustrate how can be incorporated into a media-based health-related study. We used large Twitter dataset composed of public, gender-labeled users (Dataset-1) training evaluating the...
The behaviors and emotions associated with reasons for nonmedical prescription drug use (NMPDU) are not well-captured through traditional instruments such as surveys insurance claims. Publicly available NMPDU-related posts on social media can potentially be leveraged to study these aspects unobtrusively at scale. We applied a machine learning classifier detect self-reports of NMPDU Twitter extracted all public the users. We analyzed approximately 137 million from 87,718 users in terms...
Traditional substance use (SU) surveillance methods, such as surveys, incur substantial lags. Due to the continuously evolving trends in SU, insights obtained via methods are often outdated. Social media-based sources have been proposed for obtaining timely insights, but leveraging data cannot typically provide fine-grained statistics about subpopulations, unlike traditional approaches. We address this gap by developing automatically characterizing a large Twitter nonmedical prescription...
The ability to generate and use registries—lists of patients with specific conditions, medications, or test results—is considered a measure physicians’ engagement electronic health record systems proxy for high-quality care. We conducted pre-post survey registry capability among physicians participating in the Massachusetts eHealth Collaborative, four-year, $50 million information technology program. Physicians who participated program increased their some types registries—specifically,...
The National Association of Medical Examiners convened a panel to create position paper for recommendations the investigation infant deaths due inflicted head trauma. correct certification both cause and manner death is dependent upon an evaluation all available data including information derived from investigation, scene, postmortem examination, ancillary studies. This provides forensic pathologist on what constitutes dataset be produced during examination infants who have died of, or...
Gender inequity in academic medicine is a pervasive challenge. Recommendations have been implemented to reduce inequities for female faculty. However, there are no well-established guidelines the recruitment and retention of residents. To address challenges faced by physicians support residents, emergency residents attending at Hospital University Pennsylvania formed professional development group (PDG), #Shemergency, July 2017. From 2017 2018, this PDG developed events initiatives that...

This case study review assesses the applicability of forensic fractography bone to casework through evaluation four blunt-trauma cases. The objectives were determine (1) whether fractographic features are readily identifiable in real-world trauma scenarios and (2) analysis corresponds traditional anthropological fracture pattern analysis. Fractography is surface their relationship crack propagation. Recently, diagnostic value expression variation evaluated found be informative on samples...
