We present the first case of low-grade endometrial stromal sarcoma (ESS) invading a leiomyoma, which was difficult to diagnose preoperatively. 平成8年6月に河内長野市で,翌7月に堺市で腸管出血性大腸菌O-157による多数の出血性腸炎患者が発生し,それに合併した溶血性尿毒症症候群 (HUS) 計16例を我々は今回経験した。その内訳は,男児8例,女児8例で,年齢は1歳から10歳で,HUSの完全型 (溶血性貧血,血小板減少,急性腎機能障害の3主徴がそろっているもの) 9症例と不完全型 (3主徴の1つ,ないし2つを欠くもの) 7症例であった。治療として,10例にγ-グロブリン製剤を投与した。5例 (男児2例,女児3例でいずれも完全型) に血液透析を行い,そのうちの3例には血漿交換を併用した。いずれも15病日までには透析から離脱した。HUSのその他の随伴症状として膵炎,中枢神経症状,眼底出血,トランスアミナーゼの上昇を認めた例があったがいずれも軽快している。透析を施行した症例の中で2例に持続する蛋白尿を認め,腎生検を施行し,糸球体,間質に変化を認めた。今後も長期間の経過観察が必要と思われた。
