症例は80歳代女性.1年前より食道裂孔ヘルニアおよび胃粘膜下腫瘍を認めていたが,症状がなく手術希望もなかったため,近医で経過観察としていた.今回貧血を主訴に当院を紹介受診した.上部消化管内視鏡検査と上部消化管造影検査では,混合型食道裂孔ヘルニアおよび体部前壁にdelleを伴った4cm大で胃内突出型の粘膜下腫瘍を認めた.胸腹部CTでは,胃穹窿部と前庭部が縦隔内に嵌入しており,さらに胃の内腔に突出するlow density massを認めた.貧血の原因は腫瘍からの出血であると判断し,腹腔鏡下胃局所切除術およびメッシュを用いた食道裂孔ヘルニア根治術を施行した.病理組織学的検査にて腫瘍径は40mmで,紡錘形腫瘍細胞が増生しており,免疫組織染色にてKITおよびCD34陽性,核分裂像400倍10視野合計1個未満であり,超低リスクGISTと診断した.今回,腹腔鏡下で同時に根治術を実施したので,若干の文献的考察を加えて報告する.
