症例は67歳の男性で，胃癌の診断で幽門側胃切除術，D2リンパ節郭清術，Billroth I法による再建が施行され，14病日に軽快退院となった．25病日，すしを大量摂取した後に上腹部の激痛を訴え近医受診し，急性腹症にて当院紹介となった．来院時，腹部は板状硬で，CTでは上腹部を中心とした遊離ガスを認め，上部消化管穿孔の疑いにて緊急手術となった．開腹所見では，腹腔内には多量の無臭で混濁した腹水と米粒を認めた，胃は体上部大彎で長軸方向に約3 cmの裂創が認められた．同部に明らかな炎症所見や潰瘍の形成は認められなかった．また，胃十二指腸吻合部にも明らかな異常を認めなかった．破裂部を縫合閉鎖し，腹腔内を洗浄，ドレナージを行った．術後約2週の透視でも，胃十二指腸吻合部に明らかな狭窄は認められなかった．過食による過膨張のために起こった胃破裂と考えられた．幽門側胃切除後の胃破裂は報告がなく，若干の考察を加えて報告する．

食道癌術後の後縦隔再建胃管癌に対して栄養血管切離後も胃管口側を残して胃管切除し，有茎空腸再建を行うことができた2例について報告する．症例1は73歳の男性で胸部食道癌に対して右開胸食道亜全摘，後縦隔胃管再建後である．初回手術から2年4カ月後に胃管癌を指摘され，ESDを施行したが非治癒切除となり，胃管癌指摘から4カ月後，追加胃管切除を行った．上縦隔の癒着が強固であり，胃管口側端を2cm残して有茎空腸による後縦隔経路再建，胸腔内吻合術を行った．症例2は50歳の男性で胸部食道癌に対する右開胸食道亜全摘，後縦隔胃管再建後から5年6カ月後に3型胃管癌を指摘された．上縦隔の癒着が強固であり，気管，右反回神経損傷の危険があったため胃管口側端の一部を残して右開胸開腹胃管亜全摘，有茎空腸による後縦隔経路再建，胸腔内吻合術を行った．2例とも胃管口側端の血行障害をきたすことなく術後良好に経過した．
Expression levels of CENP-F mRNA were investigated in 45 surgically resected oral squamous cell carcinomas (OSCC) and 10 samples normal gingiva, using real time reverse-transcriptase polymerase chain reaction (RT PCR). The relations between clinicopathologic variables studied. mean expression level was higher OSCC tissues than gingiva. There a significant correlation the lymph node metastasis cases (p=0.024). These results suggest that with high has metastatic potential.
症例は70歳の男性.腹痛を主訴に膵癌,切除可能と診断された.膵体尾部切除術およびD2郭清ののち,GEMによる術後補助化学療法を開始した.5コース目day1に血清クレアチニンが2.05mg/dl ,尿蛋白+になり,腹部CTで両側腎腫大あったためGEMを中止した.腎生検で薬剤性腎障害,溶血性尿毒症症候群と診断した.プレドニゾロンの投与で血清クレアチニンは低下し,プレドニゾロン開始から13カ月の現在,膵癌の再発はない.
症例は65歳の男性で，1998年6月横行結腸癌に対して右半結腸切除術，D3リンパ節郭清を施行した．最終病理学診断は中分化型腺癌，ss，n3（＋），P0，H0，M（−），StageIIIbであった．原発巣切除後約3年後に肝S6に肝内胆管の拡張を伴う腫瘤を認め，大腸癌術後肝転移再発と診断し肝後区域切除術を施行した．さらに，その4年後に肝S3に新たに腫瘤を認め，前回同様肝内胆管の拡張を伴う2度目の大腸癌肝転移と診断し肝外側区域切除術を施行した．2度とも摘出標本にて肉眼的に胆管内を進展する腫瘍を認めた．病理学的には中分化型腺癌で原発巣病理所見と類似しており，最終的に肉眼的胆管内進展を伴う大腸癌肝転移と診断した．このような特徴を有する大腸癌肝転移は，腫瘍が進展した胆管を含む系統的肝切除を行い十分なsurgical marginを確保することで長期予後が期待できる症例であり，若干の文献的考察を加え報告する．
