Abstract A child's head‐sized tumor on the upper back developed in a 43‐year‐old man. In spite of an extensive operation for tumor, he died multiple metastasis 15 months after operation. Histologically, cells proliferated throughout dermal and subcutaneous regions, boxed‐in appearance was noted with silver staining. Because electron microscopic observations strongly suggested smooth muscle origin, we diagnosed this case as cutaneous leiomyosarcoma.
白血病の小児に併発した原発性皮膚アスペルギルス症の3例を報告する。症例は入院加療中の8才女児(ALL), 10才女児(AML)および3才男児(AML)で, 点滴静注針の固定包帯部位下の手背や手掌に有痛性の漿液性丘疹が出現, 数日以内に潰瘍化し中心部に黒色壊死巣を有するようになつた。直接鏡検にて二分岐性の太い菌糸を認め培養によりA. flavusを分離同定した。治療は3例ともに抗真菌剤の全身投与と局所の湿布を行つた。症例1は, 約1ヵ月後に瘢痕治癒した。症例2においては皮疹は拡大し, X線にて骨浸潤像を認めたため, 全身状態の回復をまち, 指の切断術を施行した。症例3においてはdébridementを施行するも治癒にいたらぬうちに死亡した。白血病などの基礎疾患をもつ患者に生じたアスペルギルス症は, 発症要因として局所的要因と全身要因が関与し経過予後は患者の状態により大きく左右されると思われた。
