Trauma outcome variables before and after the institution of Advanced Life Support (ATLS) program were compared for largest hospital in Trinidad Tobago from July 1981 through December 1985 (pre-ATLS) January 1986 to June 1990 (post-ATLS). A total 199 physicians ATLS trained by 1990. Outcome data analyzed all dead or severely injured patients (ISS ≥16; n = 413 pre-ATLS 400 post-ATLS). mortality decreased post-ATLS (134 vs. 279 413) throughout hospital, including ICU (13.6% 55.2% pre-ATLS)....

We ventilated separately the right and left lungs of seven dogs having thoracotomies catheters in both lower lobe veins. Two hours after atrial injection oleic acid, shunt (Qs/QT) each increased from 0.10 to 0.47. Ten minutes positive end-expiratory pressure (PEEP) was 3 13 cmH2O one lung, mean lobar Qs/QT decreased 0.06 with no change its fraction pulmonary blood flow measured by microsphere techniques. At same time other 0.48. end expiration hilum then clamped, excised quickly frozen over...

Fetal mortality after trauma is significant. This study was aimed at identifying factors responsible for this high fetal mortality. All pregnant patients admitted to the two major Toronto institutions during period of November 1991 February 1996 with an Injury Severity Score (ISS) > or = 12 were assessed. Data on age, gestation, hypotension, ISS, hemoglobin, blood transfusion, length stay, disseminated intravascular coagulation (DIC), and specific maternal injury analyzed retrospectively...

We have previously demonstrated a significant improvement in trauma patient outcome after the Advanced Trauma Life Support (ATLS) program Trinidad and Tobago. In January of 1992, Prehospital (PHTLS) was also instituted. This study assessed PHTLS program.Morbidity (length stay degree disability), mortality, injury severity score, mechanism injury, age, sex among all adult patients transported by ambulance to major hospital were between July 1990 December 1991 (pre-PHTLS, n = 332) 1994 June...

Over a 9-year period (July 1981-December 1985-pre-ATLS period; January 1986-June 1990-post-ATLS period), the hospital charts of 813 trauma patients with ISS ≥ 16 were reviewed (n = 413, pre-ATLS and n 400, post-ATLS) in order to assess impact ATLS program. The frequency endotracheal intubation (ET), nasogastric tube insertion (NG), intravenous access (IV), Foley catheterization bladder (Foley) chest (CT) compared by Pearson Chi-square analysis. Overall, vs. post-ATLS frequencies (%) 83.5...

Background Part of the senior medical students' examination at University Toronto involves testing with simulated patient management. We compared performance in these simulations students who received Advanced Trauma Life Support (ATLS) training those did not receive ATLS training. Methods Thirty-two (group I) completed a standard course, 12 II) audited and their trauma was 44 matched control III) from same class. Performance nontrauma simulation stations also analyzed. We studied the effect of furosemide on pulmonary oxygen exchange, lung liquid, and central hemodynamics in dogs with capillary leak induced by intravenous oleic acid (OA). 2 h after OA, triple indicator-dilution liquid volume shunt (Qs/Qt) doubled despite normal wedge pressure 16 compared not given OA which no variable change during same time. Six edematous were then treated (1 mg/kg), later they showed significant reductions Qs/Qt liquid. In contrast, six other increased The changes edema...

Improvement in trauma patient outcome has been demonstrated after the implementation of Prehospital Trauma Life Support (PHTLS) program Trinidad and Tobago. This study was aimed at identifying prehospital care factors that may explain this improvement. All patients transferred by ambulance to major referral hospital had assessment airway control, oxygen use, cervical (C)-spine hemorrhage as well splinting extremities during pre-PHTLS (July 1990 December 1991; n = 332) post-PHTLS periods...

In pulmonary edema, the relationship between cardiac output (QT) and shunt (QS/QT) may be due to a diffusion barrier for O2 transfer (incomplete alveolar-capillary equilibration) or redistribution of increased blood flow toward edematous units. We compared multiple inert gases in left (LLL) right (RLL) lower lobes whole lungs eight dogs having oleic acid edema LLL. When mean QT was from 3.0 5.5 l X min-1 during ventilation, relative perfusion LLL did not increase but QS/QT because 56 78%....
