症例は76歳,男性.1995年4月に右腎細胞癌に対して右腎摘除術を受けた後,徐々に腎機能が低下し,2006年2月に血液透析が導入された.前医で維持透析中,右副腎腫瘍およびエリスロポエチン抵抗性貧血が出現し,2008年5月に当科へ紹介された.副腎腫瘍は転移性と考えられた.また,MRI T2強調像で肝は著明な低信号を示し,肝ヘモクロマトーシスが疑われた.血清フェリチン値は2,314ng/mLと高値で,前医での輸血および鉄剤投与による鉄過剰症が考えられた.右副腎腫瘍摘除術(病理所見:腎細胞癌の転移)後,骨髄生検が行われた.所見は骨髄異形成症候群で,環状鉄芽球の割合が20%であったことから,鉄芽球性不応性貧血と診断された.高齢かつ複数の重篤な合併症のため,免疫抑制療法より輸血療法の安全性が高いと判断し,2008年6月より鉄芽球性不応性貧血による貧血に対しては赤血球濃厚液2単位/週の輸血,鉄過剰症に対しては経口鉄キレート剤(デフェラシロクス:エクジェイド®)投与を開始した.デフェラシロクスは500 mg/日から,血清フェリチン値が減少傾向を示すまで徐々に増量し,血清フェリチンが1,000...
