Intravenous ketamine demonstrated rapid antidepressant effects in an optimized study design, improving depression severity 64% of treatment-resistant patients 24 hours after a single dose. The double-blind trial provides evidence for the role N-methyl-d-aspartate glutamate receptor depression, not currently activated by existing drugs.

Background Preliminary evidence suggests intravenous ketamine has rapid effects on suicidal cognition, making it an attractive candidate for depressed patients at imminent risk of suicide. In the first randomized controlled trial using anesthetic control condition, we tested ketamine's acute explicit cognition and a performance-based index implicit (Implicit Association Test; IAT) previously linked to behavior. Method Symptomatic with treatment-resistant unipolar major depression (inadequate...

Ketamine produces rapid antidepressant effects in treatment-resistant depression (TRD), but the magnitude of response varies considerably between individual patients. Brain-derived neurotrophic factor (BDNF) has been investigated as a biomarker treatment and implicated mechanism action ketamine. We evaluated plasma BDNF associations with symptoms 22 patients TRD enrolled randomized controlled trial ketamine compared to an anaesthetic control (midazolam). significantly increased levels...

Electroconvulsive therapy (ECT) and subanesthetic intravenous ketamine are both currently used for treatment-resistant major depression, but the comparative effectiveness of two treatments remains uncertain. We conducted an open-label, randomized, noninferiority trial involving patients referred to ECT clinics depression. Patients with depression without psychosis were recruited assigned in a 1:1 ratio receive or ECT. During initial 3-week treatment phase, received either three times per week...

Importance The ELEKT-D: Electroconvulsive Therapy (ECT) vs Ketamine in Patients With Treatment Resistant Depression (TRD) (ELEKT-D) trial demonstrated noninferiority of intravenous ketamine ECT for nonpsychotic TRD. The purpose of this study was to evaluate the effects acute, oral modafinil (200 mg) exposure on daytime sleepiness in methamphetamine (Meth)-dependent individuals. Eighteen Meth-dependent subjects were enrolled a 7-d inpatient and administered placebo or day 6 counter-condition 7 (randomized) protocol. Subjects completed several subjective daily assessments (such as Epworth Sleepiness Scale, Pittsburgh Sleep Quality Index, Beck Depression Inventory visual analogue scale) throughout protocol...

Pain is the primary complaint and main reason for prolonged recovery after laparoscopic cholecystectomy. The authors hypothesized that patients undergoing cholecystectomy will have less pain four hours surgery when receiving maintenance of anesthesia with propofol compared to isoflurane, desflurane, or sevoflurane. In this prospective, randomized trial, 80 scheduled were assigned propofol, sevoflurane anesthesia. Our outcome was measured on numeric analog scale surgery. We also recorded...

More than eleven million U.S. Veterans are at least 65 years of age, an age group which almost 20% suffers from clinically significant depressive symptoms. Available pharmacological treatments suboptimal for patients, including veterans, with late-life depression. However, few studies have examined the safety, tolerability and efficacy ketamine older adults TRD (LL-TRD). This study uses...
