An 83-year-old man underwent transcatheter arterial chemoembolization (TACE) for a 20-mm hepatocellular carcinoma (HCC) in Couinaud's segment 4. Computed tomography (CT) 4 months after TACE showed tumor thrombus the portal vein addition to diffuse metastases and arterioportal shunts left lobe. 症例は65歳の男性.59歳より肝細胞癌に対して肝動脈化学塞栓療法(TACE)とラジオ波焼灼療法で治療していた.62歳時に門脈血栓症に対しワルファリンカリウムの内服を開始したためprotein induced by vitamin K absence or antagonist-II(PIVKA-II)は測定しなかった.64歳時の5月に肝S7に13 mm大,肝S8に12 mm大の肝細胞癌の再発を認めTACEを施行し,その後再発なく経過した.同年11月に腹部造影CTにて肝右葉(S5-6)に造影効果のない辺縁不明瞭な30 mm大の低吸収域を認めた.ワルファリンカリウムをエドキサバンに変更しPIVKA-IIを測定したところ,952 mAU/mLと高値を示した.PET-CTで同部位に集積亢進を認め,肝細胞癌の再発と診断し,低分化型肝細胞癌が比較的急速に進行したものと考えた.肝細胞癌の患者では,α-フェトプロテインとPIVKA-IIが腫瘍の進行の指標となるため,直接経口抗凝固薬の使用が望ましいと考えられた.

Pancreatic neuroendocrine carcinoma (NEC) as classified in the World Health Organization (WHO) 2010 was reclassified WHO 2017 either tumor (NET) G3 or NEC.
