A 70-year-old man with a history of food-dependent exercise-induced anaphylaxis (FDEIA) since age 50 was admitted to the emergency department chest pain and urticaria caused by FDEIA. Coronary angiography revealed total occlusion proximal left anterior descending coronary artery. After thrombus aspiration, bare metal stent placed into culprit lesion, resulting in no residual stenosis. Urticaria disappeared on second hospital day. This is first reported case, our knowledge, which acute...

The impact of left atrial (LA) size on isolation area (ISA) using a 28-mm second-generation cryoballoon (CB) in the acute phase after pulmonary vein (PVI) and differences CB from contact force-guided radiofrequency (RF) ablation have not been fully investigated. We examined 85 consecutive patients (CB group, 35; RF 50) with drug-refractory paroxysmal fibrillation who underwent their first PVI procedure at two institutions. We evaluated ISA 3D-Merge computed tomography images (GE Healthcare,...

Right ventricular septal pacing is thought to be better than right apical for shortening the QRS duration and preserving left function. However, may not effective in all cases. In this case report, we present a rare which induced thoroughly separated excitation despite presence of relatively narrow wave during atrium-only pacing. We report 63-year-old Japanese man with cardiomyopathy an implantable cardioverter defibrillator placement tachycardia. Three years after implantation, he developed...

Abstract Background Cyclic variation of heart rate (CVHR) associated with sleep‐disordered breathing reflects cardiac autonomic responses to apneic/hypoxic stress. However, the association CVHR function is unclear. Methods We investigated a total 181 patients who underwent both 24‐hour Holter electrocardiography (ECG) and quantitative gated single‐photon emission computed tomography (SPECT) myocardial functional imaging, excluding atrial fibrillation, infarction, structural disease,...

Introduction: Atrial fibrillation (AF) activates coagulation system leading to hypercoagulation of the blood. However, it is still unknown whether rapid atrial excitation per se affects gene expression remotely in liver, major source factors and other prothrombotic molecules. Methods Results: The AF model was created by pacing at frequency 1200 bpm anesthetized 10-week-old Sprague-Dawley rats. livers peripheral blood cells were collected analyzed after 12 hours. Sham-operated rats underwent...

Case 1, 46-year-old female suffered from palpitation and diagnosed as inappropriate sinus tachycardia (IST). Her total heart beats in Holter ECG revealed 12445 beats/day despite taking verapamil metprolol. We performed EnSite multielectrode array-guided electrophysiological study radiofrequency catheter ablation (RFCA). Baseline rate (HR) was 90 bpm. HR increased to 140 bpm earliest activation site (EAS) shifted cranially about 25 mm by isoproterenol (ISO) infusion. RFCA EAS repeated until...
