We herein report a case of adenomyoepithelioma (AME) the breast with ductal carcinoma in situ (DCIS) involving 71-year-old Japanese woman. She presented bloody discharge from left nipple. Mammography and ultrasonography showed well-defined polygonal tumor. Fine-needle aspiration cytology mass stamp nipple malignancy. Mastectomy sentinel lymph node biopsy were performed. 症例は51歳，女性．不正性器出血を主訴に近医を受診した．CTで骨盤内腫瘍を指摘されたため当院産婦人科を紹介．その際，胸部CTで後縦隔腫瘍を指摘され当科へ紹介．骨盤内腫瘍は成熟奇形腫，後縦隔腫瘍は嚢胞性の良性腫瘍と診断し，同時に摘出術を行った．まず腹腔鏡下に両側付属器切除術を施行し，右半側臥位に体位変換，胸腔鏡下後縦隔腫瘍摘出術を行った．後縦隔腫瘍は薄い被膜に覆われた単房性嚢胞で，慎重に破らず摘出した．周囲との連続性は無かった．病理組織検査で，骨盤内腫瘍は成熟奇形腫瘍，後縦隔腫瘍は，腫瘍内腔は線毛円柱上皮で被覆され，免疫染色で被覆上皮はエストロゲンレセプター，プロゲステロンレセプター陽性でありミュラー管嚢胞と診断された．ミュラー管嚢胞は閉経前後の女性に認める縦隔嚢胞性疾患の新しいカテゴリーである．
症例は46歳，女性．進行卵巣癌の術後化学療法中，胸部CTで左乳房に結節を指摘され，当科紹介受診となった．触診上明らかな腫瘤は触知されず，超音波検査で10×7mmの境界微細鋸歯状，内部不均一な低エコー腫瘤が認められた．穿刺吸引細胞診で，ductal carcinomaと診断されたため，乳房部分切除，センチネルリンパ節生検が施行され，組織学的に腋窩リンパ節，腫瘤ともに卵巣癌の転移であることが判った．今回われわれは極めてまれな卵巣癌の乳腺転移を経験したので，若干の文献的考察を加えて報告する．
In this paper, we report on a new technique of Dumon stent placement using flexible bronchofiberscope. This procedure was used to insert stents in two bronchial obstruction and bronchofistula cases. The stenting described here can be through either tracheostoma or the oral cavity; it presents very straightforward safe that does not require use rigid bronchoscope.
Male breast cancer is an extremely rare malignancy. We treated three male patients. All patients showed clinical N0 and received sentinel lymph node biopsy. Because the was positive for metastasis in one patient, a total mastectomy with axillary dissection performed. 症例は57歳,男性. 45歳時に急性虫垂炎の手術歴.腹痛を主訴に来院,腹部単純レントゲンにて小腸ガスを認めた.当初,癒着性イレウスを疑い,イレウス管を挿入し加療したが軽快せず,腹部CTで腸重積を疑わせる所見を認めた.大腸内視鏡を施行したところ上行結腸に回腸が嵌入しており,先進部に隆起性の病変を認めた.小腸腫瘍を先進部とする腸重積の診断で,開腹下に回盲部切除を行った.切除標本では,回盲弁から15cm口側の回腸にIp型の隆起性病変を認め,組織学的には腺腫成分を伴った高分化腺癌であった.癌は粘膜固有層に限局し,リンパ節転移も認めなかった.早期小腸癌は稀な疾患であり,近年の報告例も含め文献的考察を加え報告する.

症例は86歳，女性．左乳房の変形，ひきつれを主訴に当院を受診し，左乳癌，cT2N0M0 stage IIA，ER:3b，PgR:2，HER2:1+と診断された．高齢のため侵襲的な治療は希望されず，ホルモン療法を開始した．アナストロゾール投与開始3カ月後，腫瘍の一時的な縮小を認めたが，投与7カ月目の検査で再増大を認め，フルベストラント（FUL）に変更した．治療開始後，腫瘍は増大せず（SD），以後1年間SDを継続した．乳癌診療ガイドラインによると，高齢乳癌症例に対しても非高齢者と同様に手術療法が標準となっている．しかし，高齢・併存疾患・死生観などを理由として手術の同意が得られない患者は一定数存在する．われわれは手術の同意が得られない高齢乳癌患者に対してフルベストラントを使用し，長期のSDを維持した症例を経験したので報告する．

症例は50歳女性。49歳時から肝硬変症の既往があり, 外来通院中であった。突然の腹痛, 嘔吐を主訴に来院。臍部の膨隆ならびにCT所見より, 臍ヘルニア嵌頓と診断し緊急手術を施行した。嵌頓小腸に壊死を認めたため, 回腸切除および回腸瘻造設術を施行した。肝硬変症に合併した成人嵌頓臍ヘルニアは予後が悪いとされるが, 迅速な診断と侵襲を最小限に抑えた外科治療により救命可能であった。
