治療抵抗性の三叉神経痛に対してNMDA受容体拮抗作用を有するイフェンプロジルが有効であった3例を経験した.【症例1】原発性マクログロブリン血症で当院通院中の70歳,女性.60歳頃から三叉神経痛を自覚.カルバマゼピンは無効で,イフェンプロジルを投与したところ,2~4週間で痛みがほぼ消失した.休薬により症状が再燃したが,イフェンプロジルの再開により痛みが消失した.しかし,1年半ほどでイフェンプロジルの効果が減弱したため,プレガバリンに変更して経過観察中.【症例2】骨髄異形成症候群で当院通院中の89歳,男性.以前より三叉神経痛を訴えていたが,カルバマゼピン,神経ブロックは無効で,イフェンプロジルを投与したところ,2カ月程度で痛みが消失した.【症例3】62歳,女性.50歳代の頃から三叉神経痛を自覚.カルバマゼピンが無効で,イフェンプロジルの投与により,約1カ月で痛みが軽減した.
