Nonchordomatous lesions of the clivus are rare entities. Nasopharyngeal radium irradiation (NRI) treatment consists application to posterior nasopharynx near orifice Eustachian tubes, an area adjacent clivus. Here we present a unique case patient with history NRI 70 years prior presentation space-occupying clival lesion suspicion for skull-based malignancy.
