The authors present the case of a 17-year old man with slit ventricle syndrome, presenting as progressive neurological deterioration after head trauma. Serial computed tomography scans revealed slight ventricular enlargement, suggesting shunt malfunction. Communication between lumbar subarachnoid space and lateral ventricles was confirmed by cisternography. He underwent lumboperitoneal shunt, resulting in complete resolution symptoms. 症例は82 歳の女性．左片麻痺を認め，救急車で搬送受診．頭部CT にて明らかな異常はなく，心電図にて心房細動は認めなかった．NIHSS は18 点．頭部MRI にて，右大脳基底核に梗塞を認めた．MRA にて右中大脳動脈閉塞を認め，発症後3 時間30 分にt-PA 静脈注射を施行．t-PA 投与15 分後に左上下肢麻痺は改善したが，開始後55 分に完全失語，右片麻痺が出現した．MRA にて左中大脳動脈閉塞を認めたため，血管内手術を施行．血栓吸引し，再開通が得られた．CT にて脳出血の合併はなく，右上下肢麻痺も改善を認めた．心エコーにて明らかな心臓内血栓を認めなかったが，心電図にて心房細動を認めた．稀に，t-PA 静脈注射後に新たな脳梗塞が発生することが報告されており，いずれも予後不良である．特に心房細動を伴う患者のt-PA 静脈注射の際には脳塞栓再発の可能性も念頭におき，血管内治療の準備をしておくべきであると思われた．
