外傷性横隔膜ヘルニアの2例を経験したため報告する。症例1は76歳の男性。軽トラックを運転中に乗用車と衝突し受傷した。胸部レントゲン，CT検査にて左胸腔内に腹腔内臓器の脱出を認め，外傷性左横隔膜ヘルニアの診断にて開腹手術を施行した。左横隔膜腱中心に約12cmの損傷を認めたため縫合閉鎖を行なった。症例2は75歳男性。軽トラックを運転中に乗用車と衝突した。胸部レントゲン，CT検査で左胸腔内に腹腔内臓器の脱出を認め，外傷性左横隔膜ヘルニアの診断で開腹手術を施行した。左横隔膜腱中心に約15cmの損傷を認めたため縫合閉鎖を行なった。外傷性横隔膜ヘルニアは比較的まれな病態であるが，手術による治療を必要とするため早期の診断が重要である。また，鈍的外傷による外傷性横隔膜ヘルニアでは高エネルギー外傷を原因とすることが多く他臓器の損傷を合併しうるため，診断および手術の際にはそれらを見逃さないようにすることが重要と考えられた。

特発性大網出血に対し腹腔鏡手術を行ない救命した1例を経験したので報告する。症例は17歳男性，腹痛を主訴に当院救急外来を受診した。初診時，腹部打撲等の外傷の既往がなく便秘の診断にて帰宅した。帰宅後，腹痛が増悪し再受診，入院のうえ経過観察となった。入院後も症状が徐々に増悪したため，造影CT検査を行なったところ腹腔内出血が明らかとなり原因不明の腹腔内出血として出血部位検索，止血目的にInterventional Radiology（IVR；画像下治療）を施行した。IVRでは明らかな出血源が同定不能であったため腹腔鏡検査および止血術を行なう方針とした。腹腔内所見では大網に血腫を形成しており，外傷の既往がないことから特発性の大網出血として大網切除を行なった。術後経過は良好で第4病日退院となった。大網出血に対して低侵襲の腹腔鏡手術を行なうことで，短期間の入院で治療が可能であった。
