The bicuspid aortic valve (BAV) exists in a wide variety of phenotypes. aim this study was to assess the anatomical characteristics different phenotypes and develop classification system aid surgical repair.
We retrospectively analysed our 20-year experience on surgical treatment of native mitral valve (MV) endocarditis in a single institution using an early and repair-oriented approach. From August 1991 to December 2015, 192 consecutive patients underwent MV surgery for active endocarditis. Of these, 81% (n = 155) had repair while 19% replacement. In-hospital late outcomes were the 2 groups subgroups with without use patch. Study end points overall survival, reoperation valve-related events. The...
Abstract OBJECTIVES Our goal was to analyse the influence of preoperative aortic regurgitation (AR) on necessity cusp repair during valve-sparing reimplantation (VSR). We focused patients with tricuspid valves (TAV) and evaluated impact AR long-term outcomes. METHODS From March 1998 December 2018, a total 512 consecutive underwent VSR at our institution; these, 303 had TAV. The mean age 53 ± 15 years, median follow-up 6.12 years. rate type were analysed based AR. Time-to-event analysis...
To analyse the long-term outcomes of aortic valve (AV) repair with biological patch in patient non-rheumatic disease. From 1995 to 2011, 554 patients underwent elective repair; among them, 57 (mean age 45 ± 17 years) had cusp restoration using for Seven (12%) unicuspid valve, 30 (53%) bicuspid and 20 (35%) tricuspid valve. Autologous pericardium was used 26 (7 treated, 19 non-treated), bovine 26, autologous leaflet 4 homograft 1. Patching perforation (n = 20, 35%), commissural defect 18,...
To compare long-term outcomes after bicuspid aortic valve (BAV) repair utilizing the Cabrol annuloplasty versus sparing Reimplantation technique. From 1996 to 2018, 340 consecutive patients underwent BAV repair. Eighty and 189 Reimplantation. Exclusion criteria were re-repairs (n = 6), active endocarditis 4), no 41) ring or suture 20). We compared both groups for survival, reoperations, related events recurrent severe regurgitation (AR > 2+). Inverse probability weighting (IPW) was used...
Mitral valve (MV) repair is the gold standard for treatment of degenerative mitral regurgitation. A variety surgical techniques allow surgeons to achieve a high rate MV even with diseases other aetiologies. However, certain number repairs fail over time. The aim this study was review our single-centre experience re-repair and analyse mode failure, safety efficiency in relation initial aetiology.Between 1997 2015, 91 patients underwent redo re-repair. first performed institution 59% cases....
Abstract OBJECTIVES Our goal was to assess the aortic leaflet free margin length (FML) and geometric height (gH) in a normal valve (AV), aorta dilatation prolapse. METHODS We measured FML gH intraoperatively 132 patients operated on for insufficiency, dilatation, endocarditis or fibroelastoma. Patients were divided into 3 groups: tricuspid AV (group 1, n = 12), 2, 43, bicuspid 18) prolapse 3, 32, 27). The compared between groups leaflets within each group. RESULTS In AV, mean 34.7 ± 3.1 mm...
In some patients undergoing a valve-sparing reimplantation technique, coronary ostium may be very close to one of the commissures. This condition jeopardizes patency and valve reimplantation. The authors describe simple safe modification leaving misplaced attached commissure.
Abstract Cigarette smoke is a major risk factor for both coronary heart disease and peripheral vascular has been reported to contain many oxidizing agents that lead generation of free radicals. In this study, we investigated the levels lipid peroxides (LPO) antioxidant vitamins (C E), total thiol content (t‐SH), activities superoxide dismutase (SOD), glutathione S‐transferase (GST) peroxidase (GPx) in platelet‐rich plasma (PRP) 50 smokers 30 non‐smokers. Total cholesterol (TC), low...
It has been reported that changes in cytokine levels affect mitochondrial functions, of hypoxia-inducible factor α (HIF-1α), and tissue damage during sepsis. We aimed to investigate the effects simvastatin pretreatment on enzyme activities, ghrelin, HIF-1α, thiobarbituric acid reactive substances (TBARS) kidney Rats were separated into four groups, namely, control, lipopolysaccharides (LPS) (20 mg/kg), + LPS. measured activities TBARS using spectrophotometry. The histological structure...
We present the case of a twenty-eight-year-old male with severe aortic stenosis and mean gradient across valve (AV) 52 mmHg, due to calcified congenital unicuspid AV.The patient was asymptomatic, without significant past medical history, clean coronaries.Due extensive AV calcifications, repair not feasible, Ross procedure indicated.Echocardiography identified dilated annulus 28 mm; root ascending aorta were 34 40 mm in diameter, respectively.Following our treatment strategy for procedure,...
Aortic valve-sparing procedures are alternative options to aortic valve replacement in patients with root aneurysm and/or severe regurgitation reducing the risk of prosthesis-related complications, such as thromboembolism, and have no need for long-term oral anticoagulation. However, these techniques technically demanding results highly dependent on perfect intraoperative restoration function. We describe a systematic approach reimplantation technique way it is currently performed our institution.
Reconstructive surgery of the aortic valve is a recent development in our center, based on old roots. In 1950s and 1960s, first procedures were performed with neither adequate echocardiography nor clear understanding geometry; those early attempts limited clinical outcome. Meanwhile, availability substitutes enabled more reproducible consistent results. Nowadays, help transesophageal echocardiography, discovery limitations substitutes, repair has become an inspiration for many...
